What SHIP Actually Does

The State Health Insurance Assistance Program — SHIP — is a federally funded, state-run counseling service that helps Medicare beneficiaries and their families navigate coverage decisions at no cost. In Maine, SHIP counselors are housed within the five regional Area Agencies on Aging, which also serve as the state's Aging and Disability Resource Centers (ADRCs).

SHIP counselors are trained volunteers and staff who hold no financial ties to insurance companies. They don't sell policies. They don't earn commissions. Their job is to sit down with you — in person, by phone, or virtually — and walk through your specific Medicare situation so you understand what you're paying for and whether better options exist.

This matters because Medicare is not one program. It's a layered system of Part A hospital coverage, Part B outpatient coverage, Part D prescription drug plans, Medicare Advantage plans, and Medigap supplemental policies. Each has its own enrollment windows, penalties for late sign-up, and coverage gaps that can cost thousands of dollars annually if you choose poorly.

What a SHIP Counselor Helps With

A typical SHIP appointment covers whichever of these applies to your parent's situation:

Initial Medicare enrollment. When a parent turns 65 or becomes Medicare-eligible through disability, a SHIP counselor walks through the Initial Enrollment Period — the seven-month window around the 65th birthday — and explains the consequences of missing it. Late enrollment in Part B carries a 10% premium penalty for each full 12-month period of delayed enrollment, and that penalty is permanent.

Medicare Advantage vs. Original Medicare. Maine has a limited but growing number of Medicare Advantage plans. SHIP counselors compare plan networks, out-of-pocket maximums, and formularies against Original Medicare plus a Medigap supplement. Appeals and billing disputes. If a parent receives a denial for a medical service, hospitalization, or skilled nursing stay, SHIP counselors explain the appeals process and help draft the initial request for reconsideration.

Low-income assistance programs. Counselors screen for Medicare Savings Programs (QMB, SLMB, QI) and the Extra Help/Low-Income Subsidy program for Part D. These programs can eliminate or substantially reduce premiums, deductibles, and copays for qualifying individuals.

How SHIP Connects to Long-Term Care Decisions

SHIP counseling becomes particularly important when a parent transitions from independent living to a care setting. Medicare covers skilled nursing facility care for the first 20 days at 100% following a qualifying three-day hospital stay. From day 21 through day 100, the daily coinsurance is $217 in 2026. After day 100, Medicare pays nothing.

Maine's Five Regional SHIP Offices

SHIP services are delivered through the same Area Agencies on Aging that handle Options Counseling and caregiver support:

Southern Maine Agency on Aging (SMAA) covers Cumberland and York counties. Based in Scarborough, SMAA handles the highest counseling volume in the state given Portland's population density.

SeniorsPlus covers Androscoggin, Franklin, and Oxford counties from its Lewiston headquarters. This is the region's ADRC and handles both SHIP counseling and broader aging services.

Spectrum Generations covers Kennebec, Knox, Lincoln, Sagadahoc, Somerset, and Waldo counties. Based in Augusta with satellite offices in several smaller communities.

Eastern Area Agency on Aging (EAAA) covers Hancock, Penobscot, Piscataquis, and Washington counties from Brewer. The Downeast region's geography makes phone and virtual counseling sessions especially important.

Aroostook Area Agency on Aging (AAAA) covers Aroostook County from Presque Isle. The most geographically isolated region, where SHIP counselors often serve communities more than an hour's drive from the nearest office.

When to Schedule an Appointment

The busiest period for SHIP counselors is Medicare Open Enrollment, which runs from October 15 through December 7 each year. During this window, beneficiaries can switch Medicare Advantage plans or Part D prescription drug plans, and call volumes spike. Scheduling an appointment in September or early October, before the rush, gives your parent more time with a counselor and more flexibility in appointment slots.

Beyond Open Enrollment, schedule a SHIP appointment whenever:

  • A parent is approaching age 65 and hasn't received guidance on enrollment options
  • A parent's health has changed significantly, making their current plan's formulary or network inadequate
  • A parent has been hospitalized and you need to understand post-acute coverage limits
  • A parent is receiving bills or denial notices they don't understand
  • You're evaluating whether a parent qualifies for low-income Medicare assistance programs
