SHIP Medicare Counseling: Free Help with Medicare Enrollment

SHIP — the State Health Insurance Assistance Program — is a federally funded counseling service that provides free, unbiased Medicare help in every state. SHIP counselors don't sell insurance, earn no commissions, and have no financial incentive to steer you toward any particular plan. They exist specifically to help people navigate Medicare enrollment, compare plan options, and resolve billing problems.

If you're overwhelmed by Medicare's enrollment windows, penalty rules, and plan choices, SHIP is your first call. But understanding what SHIP can and can't do will save you from hitting a wall during a time-sensitive enrollment.

What SHIP Counselors Do

SHIP counselors are trained volunteers and staff who can help with:

  • Comparing Medicare Supplement (Medigap) plans side by side
  • Evaluating Medicare Advantage vs. Original Medicare for your situation
  • Reviewing Part D prescription drug plans to find the lowest-cost option for your specific medications
  • Explaining enrollment periods — Initial, Special, and General — and which one applies to you
  • Identifying eligibility for cost-saving programs like the Medicare Savings Program (which pays your Part B premium) and Extra Help (which reduces Part D costs)
  • Answering billing questions and helping you understand Explanation of Benefits statements
  • Filing Medicare appeals when claims are denied

Counseling is available in person, by phone, and in many states online. SHIP operates under different names in different states — it's called HICAP in California, SHINE in Florida, and other local names elsewhere — but it's the same federally funded program everywhere.

How to Find Your Local SHIP

The national SHIP locator is at shiphelp.org or by calling 1-877-839-2675. Enter your zip code and you'll get contact information for your state's SHIP office, along with local counseling locations — many operate out of Area Agencies on Aging, senior centers, and libraries.

The Limitations You Should Know

SHIP is free and valuable, but it has structural constraints:

Severe response delays. Federal SHIP funding was approximately $70 million in 2025 — roughly $1 per Medicare beneficiary. Counselors are often volunteers with limited availability. During peak enrollment periods (October through December for Annual Enrollment, January through March for General Enrollment), wait times can stretch to weeks. If you're inside an 8-month Special Enrollment Period with a hard deadline, waiting for a SHIP appointment may not be an option.

Counselors cannot submit forms for you. SHIP can explain what CMS-L564 and CMS-40B do, but they can't complete the forms, collect employer signatures, or submit applications to Social Security on your behalf. The administrative execution remains your responsibility.

No legal authority assistance. SHIP doesn't handle power of attorney, representative payee designations, or Form SSA-1696 (Appointment of Representative). If you're trying to enroll a parent in Medicare and need federal representative authority, SHIP can't help with that process.

No asset protection or Medicaid planning. SHIP covers Medicare, not Medicaid. Questions about Medicaid eligibility, estate recovery, look-back rules, or asset protection strategies are outside SHIP's scope.

SHIP vs. Insurance Brokers

Insurance brokers also offer free Medicare consultations — but "free" means commission-funded. A broker earns a recurring commission from whichever Medicare Advantage or Part D plan you choose. That doesn't automatically mean bad advice, but the incentive structure is different from SHIP's.

Where SHIP may recommend staying on Original Medicare with a Medigap supplement, a broker's compensation model favors Medicare Advantage enrollment. Neither is inherently wrong — but knowing the incentive helps you evaluate the advice.

For pure plan comparison with no sales pressure, SHIP is the better option. For hands-on enrollment assistance with a specific Advantage or Part D plan, a licensed broker can actually process the enrollment for you — something SHIP can't do.

When You Need More Than Free Help

SHIP excels at plan comparison and general Medicare education. But certain situations require more comprehensive support:

  • Employer-to-Medicare transition — coordinating CMS-L564 forms, HSA contribution cutoff dates, COBRA avoidance, and the 8-month SEP timeline
  • Enrolling a parent who can't manage their own paperwork — requires SSA-1696 representative authority, which SHIP doesn't handle
  • Penalty avoidance for complex scenarios — workers over 65, COBRA recipients, HSA contributors, and military retirees each face unique penalty traps
  • Multi-program coordination — families managing Medicare, Medicaid, VA benefits, and TRICARE simultaneously
