Every state has a federally funded program that provides free, one-on-one Medicare counseling — and most caregivers have never heard of it. The State Health Insurance Assistance Program (SHIP) puts you in front of a trained counselor who can help compare plans, explain coverage, review denials, and screen for cost-saving programs. No sales pitch, no commission, no cost.
SHIP counselors are the only Medicare advisors who aren't selling something. Unlike insurance brokers — who earn commissions that can reach $750 per Medicare Advantage enrollment and create structural incentives to steer your parent toward specific plans — SHIP counselors are prohibited from recommending specific insurers or plans by name.
What SHIP Counselors Actually Do
SHIP services cover the full range of Medicare questions caregivers face:
Plan comparison during enrollment periods. During the Annual Election Period (October 15 through December 7) and other enrollment windows, SHIP counselors will sit down with your parent's medication list, provider preferences, and budget to walk through available Medicare Advantage, Medigap, and Part D plans. They use the same Medicare.gov plan finder tools but provide the contextual knowledge to interpret the results.
Benefits screening. SHIP counselors can check whether your parent qualifies for Medicare Savings Programs, Extra Help/Low-Income Subsidy for Part D, state pharmaceutical assistance programs, and other cost-reduction benefits — often in a single appointment.
Claims and billing review. If your parent receives a confusing Medicare Summary Notice or an unexpected bill, a SHIP counselor can review the document, explain what Medicare paid versus what the provider is billing, and help identify errors.
Appeal guidance. SHIP counselors can help draft appeal letters, explain the five-level Medicare appeals process, and guide you through the documentation needed to challenge a denial. They won't file the appeal for you, but they'll help you understand what to include and where to send it.
Fraud reporting. If the Medicare Summary Notice shows services your parent didn't receive, a SHIP counselor can help report suspected fraud to the HHS Office of Inspector General.
How to Find Your SHIP
Every state runs SHIP under a different name. In California, it's HICAP (Health Insurance Counseling and Advocacy Program). In Illinois, it's SHIP. In Texas, it's HICAP. In New York, it's HIICAP. The name changes but the service is the same.
To find your state's SHIP:
- Call 1-877-839-2675 (the national SHIP hotline)
- Visit shiphelp.org for a state-by-state directory
- Call 1-800-MEDICARE and ask for your local SHIP
Most SHIPs offer counseling by phone, in person at local offices (often at Area Agencies on Aging, senior centers, or libraries), and increasingly by video. Some states have walk-in hours; others require appointments.
When to Contact SHIP
Before the Annual Election Period (AEP). SHIP offices get overwhelmed during AEP. If your parent needs a plan review, call in September to schedule an October appointment. Waiting until November means you may not get an appointment before the December 7 deadline.
After a hospital discharge. If your parent is transitioning from hospital to skilled nursing facility or home and you have questions about what Medicare covers, SHIP can explain the rules — including the three-day inpatient requirement, observation status implications, and coverage limits.
When you receive a denial. The sooner you contact SHIP after a claim denial, the more time you have to prepare an appeal within the filing deadline.
When screening for financial assistance. If your parent's income has changed or you suspect they qualify for premium assistance, SHIP counselors can run eligibility checks across multiple programs simultaneously.

Limitations to Know
SHIP is a counseling service, not a legal service. Counselors can explain options and help with paperwork, but they cannot:
- Recommend specific insurance companies or plans
- File appeals or legal documents on your behalf
- Provide legal advice about Power of Attorney, guardianship, or estate planning
- Guarantee outcomes for coverage disputes
SHIP programs are also chronically understaffed. Many rely on volunteers, and appointment availability varies widely by state and season. During peak enrollment periods, wait times can stretch to weeks — which is why calling early matters.
