Senior Health Insurance Program Illinois: Free Medicare Counseling and How to Get an Appointment

What SHIP Counselors Do

The Senior Health Insurance Program (SHIP) is Illinois's free, unbiased Medicare counseling service, run by the Illinois Department on Aging through a network of volunteer counselors at local Area Agencies on Aging (AAAs) across the state. SHIP counselors help with:

  • Comparing Medicare Advantage and Medigap plans during Open Enrollment
  • Determining eligibility for Medicare Savings Programs (QMB, SLMB, QI-1) and Extra Help
  • Understanding Medicare billing statements and Explanation of Benefits notices
  • Resolving coverage denials and explaining appeal rights
  • Navigating the transition from employer coverage or Medicaid to Medicare

SHIP counselors are trained and certified but not licensed insurance agents. They don't sell insurance, don't receive commissions, and don't steer anyone toward a particular plan or carrier. This makes them one of the only genuinely unbiased resources in a Medicare landscape dominated by broker ads and lead-generation sites.

How to Reach a SHIP Counselor

Statewide SHIP hotline: 1-800-252-8966

Call this number to be connected with your nearest SHIP site or to schedule an appointment. Counseling is available by phone, in person at local AAA offices, and at some senior centers and libraries that host periodic SHIP events.

Each of Illinois's 13 Area Agencies on Aging coordinates SHIP volunteers in their region. Rural downstate counties have fewer counseling sites and longer wait times than the Chicago metro area, but phone counseling is available statewide.

The Wait-Time Problem

During peak enrollment periods — Medicare Open Enrollment (October 15–December 7) and the months following January's Social Security COLA adjustments — SHIP appointment wait times regularly stretch to four to six weeks. The program relies heavily on trained volunteers, and demand consistently outpaces supply.

If you're facing a time-sensitive issue — a 10-day IDHS verification deadline, an appeal filing window, or an improper billing situation — waiting a month for a SHIP appointment may not be practical. In those cases:

  • Illinois Legal Aid organizations provide free assistance with MSP denials and appeals: Legal Aid Chicago (312-341-1070), Prairie State Legal Services (1-800-531-7057), and Land of Lincoln Legal Aid (1-800-252-8629)
  • The IDHS Customer Helpline (1-800-843-6154) handles case status questions and can connect you with your county FCRC
  • 1-800-MEDICARE (1-800-633-4227) takes complaints about improper QMB balance billing

What SHIP Cannot Do

SHIP counselors provide information and guidance, but they operate within clear boundaries:

  • They cannot act as an authorized representative on your case with IDHS — only a person designated through Form HFS 2998 or the ABE portal can receive official notices and speak with caseworkers on your behalf
  • They cannot submit ABE portal applications or upload documents for you
  • They cannot provide legal representation at a fair hearing
  • They cannot draft legal dispute letters for billing violations

These limitations matter most for adult children managing a parent's benefits remotely. SHIP counselors can explain what needs to happen, but the family still has to execute the paperwork, portal submissions, and follow-ups.

Making the Most of a SHIP Appointment

Come prepared with:

  • Your parent's Medicare card (red, white, and blue card showing Part A and Part B dates)
  • The most recent Social Security benefit statement showing monthly payment amount
  • Any state notices — especially Form HFS 267M verification requests or denial letters
  • Bank statements if you plan to discuss MSP eligibility (the resource limits are $9,950 single / $14,910 couple in 2026)

A well-prepared 30-minute SHIP session can confirm program eligibility, identify the right application pathway, and flag potential issues before they become problems.

Beyond SHIP: A Complete Navigation System

For families who need step-by-step execution guidance — income worksheets, document checklists, ABE portal walkthroughs, billing dispute templates, and renewal tracking calendars — the Illinois Medicare Savings Programs & Extra Help Guide covers every stage from eligibility check through post-approval premium refund. SHIP counselors are excellent for plan comparisons and eligibility screening; the guide fills the gap on administrative execution.
