The Relief That Saves Families Thousands Every Year
Nursing home fees are a qualifying medical expense under Irish tax law, and most families either don't know this or assume it only applies if they're paying privately. It applies whether your parent is on Fair Deal or paying full private rates — the claimable amount just differs.
The relief works at your marginal rate of income tax. For higher-rate taxpayers, that means 40% of qualifying fees comes back. On annual nursing home costs of €50,000 or more, the numbers add up fast.
What Qualifies for Relief
Private fees paid before Fair Deal approval: If you're covering the full weekly nursing home rate during the bridging period (the gap between admission and HSE approval), qualifying nursing-home expenses paid during that period may be claimed as medical expenses. For a family paying €1,200 per week privately for six weeks, that's €7,200 in potentially qualifying expenses — and €2,880 back at the 40% rate if the claim is eligible.
The resident's weekly Fair Deal contribution: Once Fair Deal kicks in, the applicant pays a calculated weekly contribution based on their income and assets. That contribution is a medical expense too. If your parent's weekly contribution is €350, that's €18,200 per year in qualifying medical expenses.
Out-of-scope fees: Additional charges for services outside the Fair Deal scheme — hairdressing, social activities, transport, and certain therapies — are not automatically qualifying medical expenses. Ask Revenue whether a particular extra is an eligible health expense.
Who Can Claim
The resident themselves can claim the relief. A person who pays qualifying expenses on behalf of the resident may also be able to claim, but should confirm with Revenue how the payment must be reported.
This matters during the bridging period when adult children are covering the full private rate. Each paying family member claims the portion they actually paid. The claim goes on their personal tax return using the Med 1 form (Health Expenses — Claim for Relief).

How to File the Claim
You have two options:
Through Revenue's myAccount: Log in, go to "Review your tax" for the relevant year, select "Health Expenses" and enter the qualifying amounts. You can claim online for the current year and up to four previous years.
Paper Med 1 form: Download it from revenue.ie, fill in Section 3 (nursing home fees), and submit to your local Revenue office. Attach receipts if requested — Revenue can ask for proof, though they don't require receipts with the initial filing.
The claim deadline is four years from the end of the tax year. So for fees paid in 2026, you have until the end of 2030 to claim. But filing promptly means you get the refund sooner — Revenue typically processes electronic claims within a few weeks.
The Interaction With Fair Deal
A common question: does claiming tax relief on nursing home fees affect the Fair Deal financial assessment? No. Revenue tax credits and refunds are not treated as assessable income under the Nursing Homes Support Scheme. The relief operates entirely within the tax system — it doesn't change the HSE's calculation of what your parent owes.
This means there's genuinely no downside to claiming. The tax relief reduces the net cost of care without creating any complications for the Fair Deal assessment.
