Nursing Home Costs Before Fair Deal: Bridging the Gap in Ireland
The Bill That Catches Every Family Off Guard
Fair Deal funding is not backdated. That single fact creates the most financially painful period in the entire nursing home process — the weeks (sometimes months) between your parent entering a nursing home and the HSE formally approving their application. During that gap, you pay the full private rate out of pocket.
Private nursing home fees in Ireland typically run between €1,000 and €1,500 per week, depending on the facility, location, and level of care. A six-week processing window — which is the HSE's own estimate for a straightforward application — means €6,000 to €9,000 in upfront costs before any state support begins.
For families dealing with a crisis admission after a hospital fall or stroke, there's rarely time to complete the paperwork before the parent is placed. The discharge coordinator needs a plan within days. The Fair Deal application takes weeks. That mismatch is where the bridging gap lives.
How Long the Gap Actually Lasts
The HSE processing timeline has two stages to plan for:
Document gathering and preparation — typically 2 to 4 weeks. This includes assembling six months of bank statements, pension statements, a property valuation, Revenue Notice of Assessment, and various Social Protection records. Start this while the clinical assessment is being arranged; the financial assessment cannot begin until the documents arrive at the local Nursing Homes Support Office (NHSO).
HSE administrative processing — typically 4 to 6 weeks once the NHSO receives a complete application. The care needs assessment comes first, the financial assessment follows approval, and Nursing Home Loan processing may take longer.
In practice, families who start gathering documents while simultaneously submitting the care needs assessment can compress the total timeline. But complications — disputed property titles, overseas pensions, missing tax records — stretch it further. Some families report waiting 8 to 12 weeks.
How to Reduce the Net Cost
The transition period isn't as expensive as the headline numbers suggest, once you factor in the tax relief that most families overlook.
Med 1 tax relief: Nursing home fees qualify as deductible medical expenses under Revenue's Med 1 scheme. You can claim relief at your marginal tax rate — up to 40% for higher earners. On €9,000 of private fees, that's a potential €3,600 back. The claim applies to fees paid by the resident or by a family member on their behalf. You submit the Med 1 form (available on revenue.ie) with your annual tax return.
Pension income continues: Your parent's State Pension, occupational pension, and any other income doesn't stop during the bridging period. For many families, a parent's combined pension income covers a substantial portion of weekly fees even before Fair Deal begins.
Negotiate the admission terms: Some nursing homes will accept a payment plan or agree to a reduced initial rate when they know a Fair Deal application is in progress. This isn't universal, and a pending Fair Deal application does not guarantee long-term funding.

Practical Steps to Manage the Gap
Start the Fair Deal paperwork before it's urgent. If your parent's health is declining gradually, begin gathering financial records now — the document assembly is the part you can control. Having a complete package ready helps keep administrative processing within the typical 4-to-6-week range.
If the admission is sudden (hospital discharge), ask the hospital's medical social worker to initiate the care needs assessment while your parent is still an inpatient. The clinical assessment can happen in the hospital setting, which removes one waiting step.
Keep every receipt and record of private payments during the gap. Beyond the Med 1 tax claim, these records matter if there's any dispute later about when Fair Deal funding should have commenced.
Our Fair Deal filing toolkit includes a transition cash-flow estimator that maps the bridging period week by week — showing exactly how much you'll need based on your parent's income, the facility's rate, and the tax relief you can claim. It takes the guesswork out of a period where families are already stretched thin.
