Louisiana QMB Benefits Explained — What Does QMB Cover?

What QMB Covers in Louisiana

Qualified Medicare Beneficiary status in Louisiana covers every cost-sharing obligation that Original Medicare passes to the patient. That means the Louisiana Department of Health pays your Medicare Part B premium ($202.90 per month in 2026), your Part A premium if you don't have premium-free Part A, the Part B annual deductible ($283 in 2026), the Part A hospital inpatient deductible ($1,736 per benefit period), and all coinsurance and copayments on Medicare-covered services.

In practical terms, a QMB enrollee in Louisiana owes nothing out of pocket for any service that Medicare approves. The state pays the portion that Medicare doesn't cover, and providers are legally prohibited from billing you for the difference.

This applies whether you see a doctor in Baton Rouge, get lab work in Shreveport, or have surgery at a hospital in New Orleans. If Medicare covers the service, QMB covers the cost-sharing.

QMB Copays, Deductibles, and Coinsurance — All Waived

Here's the specific breakdown of what Louisiana QMB eliminates:

Part B premium: The standard $202.90 monthly premium stops being deducted from your Social Security check once the state buy-in file reaches SSA. If premiums were deducted after your QMB effective date, SSA issues a retroactive refund.

Part B deductible: The $283 annual deductible that Original Medicare charges before it starts paying 80% of outpatient services — QMB covers it entirely. You owe nothing toward it.

Part A hospital deductible: Each time you're admitted to the hospital, Medicare charges a $1,736 deductible per benefit period. QMB pays this for you.

20% coinsurance on outpatient services: After the Part B deductible, Original Medicare pays 80% of the approved amount for doctor visits, outpatient procedures, durable medical equipment, and other Part B services. The remaining 20% coinsurance is QMB's responsibility — not yours.

Part A coinsurance for hospital stays: For hospital stays beyond 60 days, Medicare charges daily coinsurance ($434 per day for days 61–90 in 2026). QMB covers these amounts.

Skilled nursing facility coinsurance: After day 20 in a skilled nursing facility, Medicare charges $217 per day in 2026. QMB pays this coinsurance through day 100.

QMB vs. SLMB and QI Coverage

The coverage distinction matters. SLMB and QI in Louisiana pay only your Part B premium — nothing else. You remain responsible for all deductibles, coinsurance, and copayments under those programs.

QMB is the only Medicare Savings Program tier that eliminates cost-sharing entirely. If your income is at or below 100% of the federal poverty level ($1,350 per month for an individual in 2026), QMB provides the most comprehensive protection.

All three programs — QMB, SLMB, and QI — automatically trigger Extra Help with Medicare prescriptions, which reduces Part D drug costs to nominal copayments.

The Balance Billing Protection That Comes with QMB

Federal law under 42 U.S.C. § 1396a(n)(3)(B) makes it illegal for any Medicare provider to bill a QMB enrollee for Medicare deductibles, coinsurance, or copayments. This applies to every Medicare-enrolled provider in Louisiana, even if that provider doesn't participate in Louisiana Medicaid.

Louisiana uses a "lesser-of" reimbursement method. When Medicaid compares the Medicare payment to the state Medicaid fee schedule and the Medicare payment is higher, Medicaid pays $0. Providers sometimes misread that $0 Medicaid payment as permission to bill the patient. It is not. The service is paid in full under federal law, and the provider cannot collect from you.

If you receive a bill for Medicare cost-sharing while enrolled in QMB, you have the right to dispute it. The detailed process for handling illegal QMB balance bills in Louisiana involves notifying the provider in writing and escalating to 1-800-MEDICARE if needed.

How to Get QMB Coverage in Louisiana

QMB eligibility in Louisiana requires Medicare Part A entitlement and countable monthly income at or below $1,350 for an individual or $1,824 for a couple in 2026. Louisiana has eliminated the asset test for QMB — your savings, home, car, and retirement accounts are not counted.

You apply through the Louisiana Department of Health using Form 1-MB, online at MyMedicaid.la.gov, or at a local Medicaid office. The full application process is covered in our guide to applying for the Medicare Savings Program in Louisiana.
