Louisiana Extra Help Medicare Prescriptions
What Extra Help Covers
Extra Help — formally called the Part D Low-Income Subsidy (LIS) — reduces what you pay for Medicare prescription drug coverage. Full Extra Help covers the Part D plan premium (for benchmark plans) and eliminates or sharply reduces copays for covered medications.
In 2026, the Inflation Reduction Act caps total out-of-pocket Part D costs at $2,100 per year for all beneficiaries. But for Extra Help enrollees, copays are so low that most never approach that cap.
If you qualify for Extra Help but haven't enrolled in a Part D plan, CMS automatically assigns you to a benchmark plan. You can switch to a different plan during any enrollment period without losing the subsidy.
Two Paths to Extra Help in Louisiana
Path 1: Apply through SSA. You can apply directly to Social Security for Extra Help using Form SSA-1020 or at ssa.gov. In 2026, SSA's countable-resource limit is $16,590 for an individual or $33,100 for a married couple, plus an exclusion of up to $1,500 per person for burial expenses when applicable. Your income must be below 150% FPL. If your countable resources exceed the applicable limit, SSA denies the application.
Path 2: Get approved for an MSP through LDH. When Louisiana's Department of Health approves you for QMB, SLMB, or QI, it automatically reports your status to CMS each month. CMS deems you eligible for full Extra Help without any separate application — and without any asset test. Louisiana waives the resource limit for all three MSP tiers.
This distinction matters enormously. A Louisiana resident with $40,000 in a savings account would be denied Extra Help through SSA. But if their monthly income is under $1,816 (single) or $2,455 (couple), they can qualify for an MSP through LDH, which automatically triggers Extra Help. Same person, same finances, completely different outcome depending on which door they walk through.
How the Automatic Pathway Works
Each month, LDH transmits a data file to CMS listing every active MSP enrollee. CMS processes these files and updates each beneficiary's LIS status in the Medicare system. This is called "deemed" eligibility — you're considered eligible based on your MSP enrollment alone.
The timing creates a brief gap. After LDH approves your MSP application, the deemed LIS status may take one to two billing cycles to appear in the pharmacy system. During that window, you might show up at the pharmacy and be charged full price.
If this happens, the pharmacy can use the Best Available Evidence (BAE) policy to give you reduced copays immediately. Show them your LDH MSP approval notice, a MEDS printout, or any documentation of active Medicaid status. Alternatively, the pharmacist can process your claim through the LI NET program (administered by Humana) for immediate coverage while the system catches up.

What Extra Help Costs Look Like
With full Extra Help in 2026, you pay:
- $0 Part D plan premium for benchmark plans
- Nominal copays for covered drugs — low flat rates for generics and brands
- $0 copays once you reach out-of-pocket thresholds
Without Extra Help, a Part D plan premium can run $30–$60/month, and cost-sharing before the $2,100 annual cap can be substantial.
Louisiana SenioRx Is Not Extra Help
Louisiana operates a program called SenioRx through the Governor's Office of Elderly Affairs and parish Councils on Aging. SenioRx is not a state prescription assistance program — it's a navigation service that helps seniors apply to pharmaceutical manufacturer Patient Assistance Programs for free or discounted brand-name medications.
SenioRx is useful as a supplement, but it doesn't replace Extra Help. It doesn't pay Part D premiums or reduce pharmacy copays. If you qualify for an MSP, the automatic Extra Help pathway through LDH provides broader and more reliable prescription cost relief.
