Mississippi Extra Help Medicare: Part D Low-Income Subsidy
The Automatic Link Between MSPs and Extra Help
When Mississippi's Division of Medicaid approves you for QMB, SLMB, or QI, you are automatically deemed eligible for Full Extra Help — the federal Part D Low-Income Subsidy. No separate application to SSA needed, no additional paperwork, no waiting for another determination.
DOM transmits your eligibility data to CMS, which coordinates with SSA to assign deemed LIS status. Once active, Extra Help covers:
- $0 monthly Part D plan premiums (for benchmark plans)
- $0 annual Part D deductible
- Reduced prescription copayments: $5.10 for generics and $12.65 for brand-name drugs in 2026
Why Applying Through DOM Is Better Than Going to SSA
This is the detail that catches most families. If you apply for Extra Help directly through SSA, the federal agency enforces its own resource limit: $16,590 for an individual and $33,100 for a couple in 2026, before any burial exclusion. If an applicant expects to use resources for burial expenses, SSA excludes up to $1,500 per person. Bank accounts, retirement funds, and other countable resources are examined.
But Mississippi has no asset test for its Medicare Savings Programs. So a senior with $40,000 in countable savings will exceed the standalone Extra Help resource limit, but can still qualify for QMB through DOM — and that QMB approval automatically activates Full Extra Help, without applying the Extra Help resource test.
The takeaway: if your parent's income qualifies for any Mississippi MSP, apply through DOM first. The Extra Help comes along for free.
What Extra Help Saves in 2026
The Inflation Reduction Act capped Part D out-of-pocket costs at $2,100 for all Medicare enrollees in 2026, and eliminated the coverage gap ("donut hole"). But for seniors with Full Extra Help, the savings go further:
- No monthly premium on a benchmark Part D plan
- No annual deductible ($615 in 2026 without Extra Help)
- Copayments of $5.10/$12.65 instead of 25% coinsurance on brand drugs
- The $2,100 cap still applies as a backstop, but most Extra Help enrollees never reach it
For a senior taking three brand-name medications, the difference between standard Part D cost-sharing and Extra Help copayments can exceed $1,500 per year — on top of the Part B premium savings from the MSP.

When the Pharmacy Doesn't Show Extra Help Status
After MSP approval, it can take several weeks for CMS systems to update your parent's Part D plan with their Extra Help status. If they go to fill a prescription and the pharmacy charges full price, two tools fix it immediately:
Best Available Evidence (BAE): Present a copy of the DOM approval notice or the Medicaid card showing QMB/SLMB status directly to the pharmacy. Under CMS rules, the Part D plan must adjust copayments to Extra Help levels on the spot.
LI NET Program: If your parent isn't yet enrolled in a Part D plan but has retroactive MSP eligibility, claims can be processed through the Limited Income NET program (administered by Humana). LI NET provides immediate point-of-sale coverage with $0 deductibles and standard LIS copayments.
Mississippi Has No State Pharmacy Assistance Program
Unlike some states that run their own drug discount programs, Mississippi has no general senior State Pharmaceutical Assistance Program (SPAP). That makes Extra Help through the MSP-to-LIS pathway the primary route for prescription cost relief. Other options include 340B pricing at Federally Qualified Health Centers and manufacturer Patient Assistance Programs, but these operate outside the Part D benefit structure.
The Mississippi Medicare Savings Programs & Extra Help Guide explains how to maximize prescription savings through the automatic Extra Help pathway and what to do if your pharmacy doesn't recognize the benefit.
