Extra Help Medicare Maryland: How to Get the Part D Low-Income Subsidy

What Extra Help Gives Maryland Seniors

Extra Help — officially the Part D Low-Income Subsidy (LIS) — is a federal benefit that dramatically reduces prescription drug costs for Medicare beneficiaries with limited income. Since the Inflation Reduction Act changes, there's only Full Extra Help. Partial Extra Help no longer exists.

Full Extra Help provides:

  • $0 monthly premium on any benchmark Part D plan or qualifying Medicare Advantage drug plan
  • $0 annual drug deductible (otherwise $615 in 2026)
  • Minimal copays for covered medications at the pharmacy counter
  • No coverage gap — the donut hole is eliminated entirely in 2026

For a Maryland senior paying $40–$80/month in Part D premiums and hitting the deductible every January, Extra Help removes both costs completely.

Two Paths to Extra Help in Maryland

Path 1: Through a Medicare Savings Program (automatic). When Maryland approves your parent for QMB, SLMB, or QI, CMS automatically deems them eligible for Full Extra Help. No separate application. CMS sends a "purple notice" confirming the benefit. This is the most common way Maryland residents end up with Extra Help.

Path 2: Through Social Security (standalone application). Your parent can apply directly to SSA for Extra Help by filing Form SSA-1020 online, by phone, or at a Social Security field office. Eligibility requires income up to 150% of the Federal Poverty Level and limited resources.

These two paths interact. When someone applies for Extra Help through SSA, the application gets transmitted to Maryland's Medicaid agency under MIPPA rules. Maryland must then screen that person for MSP eligibility — using the SSA filing date as the protective filing date. If the person qualifies for an MSP, they get that too.

Going the other direction: an MSP application that results in approval automatically triggers Extra Help without going through SSA.

The Practical Impact

The connection between MSPs and Extra Help means most Maryland residents who qualify for one end up with both. The combined effect:

  • Part B premium eliminated (through the MSP)
  • Part D premium eliminated (through Extra Help)
  • Drug deductible eliminated (through Extra Help)
  • Pharmacy copays reduced to nominal amounts (through Extra Help)
  • For QMB beneficiaries, all deductibles and coinsurance eliminated too

A senior who was paying $202.90/month for Part B, $50/month for Part D, and $615/year in drug deductibles sees over $3,600/year in savings.

When Extra Help Ends

If your parent loses MSP enrollment, Extra Help does not end immediately. Medicare says it continues through December 31 of the year it was granted, even if income changes during that year. Medicare or Social Security will determine eligibility for the next year; read any notice and apply directly to SSA if your parent still meets the Extra Help rules.

If the notice says your parent will no longer receive Extra Help next year, SPDAP may be an option for premium relief if they meet its rules and income is under 300% FPL.

Extra Help also provides a once-per-month Special Enrollment Period to enroll in Original Medicare with a standalone Part D plan or switch between standalone Part D plans. The dual/LIS SEP does not allow a switch to a non-D-SNP Medicare Advantage plan; full-benefit duals may have a separate Integrated Care SEP for an integrated D-SNP.

The Maryland Medicare Savings Programs, Extra Help & SPDAP Guide maps the eligibility overlaps and shows which applications to file based on your parent's income level.
