Louisiana Medicare Assistance Programs 2026 — Every Cost-Help Option
The Full Landscape of Medicare Cost Help in Louisiana
Louisiana has multiple programs that reduce Medicare costs for seniors and disabled beneficiaries, but they're run by different agencies, have different eligibility rules, and cover different things. No single program does everything, and no single office coordinates them all. Here's what actually exists in 2026, what each one covers, and where to apply.
Medicare Savings Programs (QMB, SLMB, QI)
These are the biggest dollar-value programs for most Louisiana seniors. They're administered by the Louisiana Department of Health through Medicaid and directly reduce what you pay for Medicare:
QMB (Qualified Medicare Beneficiary) — for individuals with income up to $1,350/month or couples up to $1,824/month. Pays your Part B premium ($202.90/month), Part A premium if you don't have premium-free Part A, and all Medicare deductibles, coinsurance, and copayments. Providers cannot bill you for any Medicare cost-sharing.
SLMB (Specified Low-Income Medicare Beneficiary) — income up to $1,616/month individual, $2,184/month couple. Pays your Part B premium only.
QI (Qualifying Individual) — income up to $1,816/month individual, $2,455/month couple. Pays your Part B premium only. Funded through a capped federal block grant and allocated first-come, first-served.
Louisiana has no asset test for any of these three programs. Your savings, home, car, and retirement accounts don't count. Only monthly income matters, and LDH applies a $20 general income disregard before comparing your income to the limit.
Apply at MyMedicaid.la.gov, by mail using Form 1-MB, or at a regional Medicaid office.
Extra Help (Part D Low-Income Subsidy)
Extra Help reduces what you pay for Medicare prescription drugs — premiums, deductibles, and copayments on your Part D plan. In 2026, full Extra Help means $0 premium on benchmark Part D plans and nominal copayments for covered drugs.
The fastest path to Extra Help in Louisiana is through the Medicare Savings Programs. Enrollment in QMB, SLMB, or QI automatically triggers full Extra Help — no separate application needed. CMS deems you eligible based on LDH's monthly data transmission.
You can also apply directly to SSA, but SSA enforces a federal asset test ($16,050 individual, $32,110 couple) that Louisiana's MSPs waive entirely. If a parent has savings above those federal limits, apply through LDH for an MSP first — approval triggers deemed Extra Help status regardless of assets.

SHIIP — Free Medicare Counseling
The Senior Health Insurance Information Program (SHIIP) provides free, unbiased Medicare counseling through the Louisiana Department of Insurance. SHIIP counselors help you understand which programs you qualify for, compare Part D plans, resolve billing disputes, and navigate the application process.
Call 1-800-259-5300. SHIIP is informational — counselors explain your options but don't file applications or represent you at hearings.
SenioRx — Prescription Application Assistance
Louisiana SenioRx is not a state drug benefit. It's a free service through local Councils on Aging that helps seniors apply to pharmaceutical manufacturer Patient Assistance Programs for free or discounted brand-name medications. Contact your parish Council on Aging or call GOEA at (225) 342-7100.
Medicare Prescription Payment Plan (MP3)
New in 2025 and continuing through 2026, MP3 lets Medicare beneficiaries spread their out-of-pocket costs for covered Part D drugs into monthly installments over the plan year. Combined with the Inflation Reduction Act's $2,100 annual cap on out-of-pocket costs for Part D-covered prescription drugs in 2026, this provides a ceiling on those covered drug costs for Medicare beneficiaries with Part D coverage — not just low-income ones. Full Extra Help recipients rarely reach the $2,100 cap, but partial subsidy recipients and those just above MSP income limits benefit significantly.
Councils on Aging — Parish-Level Help
Every Louisiana parish has a Council on Aging that connects seniors with available programs — not just SenioRx but also Meals on Wheels, transportation, caregiver support, and benefit application assistance. They're often the best starting point when you don't know which programs apply to your parent's situation.
Legal Aid for Denials and Terminations
If an MSP application is denied or coverage is terminated incorrectly, free legal help is available through Southeast Louisiana Legal Services (1-844-244-7871) and Acadiana Legal Service Corporation (1-866-275-2572). These organizations can represent eligible low-income seniors at administrative fair hearings before the Division of Administrative Law.
What's Not Available in Louisiana
Louisiana does not have a general State Pharmacy Assistance Program (SPAP) that directly subsidizes drug costs for seniors. It does not have a state-funded Medicare premium assistance program separate from the federal MSPs. And the Medicaid Estate Recovery Program does not apply to standalone MSP benefits — the state cannot recover costs for Medicare premiums or cost-sharing paid through QMB, SLMB, or QI.
