Louisiana Medicaid Office Locations — Medicare Savings Help
Regional Medicaid Offices
The Louisiana Department of Health operates nine regional Medicaid offices that serve all 64 parishes. These offices have civil service eligibility staff who make the actual decisions on MSP applications — they're the only locations where an in-person applicant interacts with the people who approve or deny cases.
Regional offices handle applications for QMB, SLMB, QI, and all other Medicaid programs. Walk-in availability varies by location and season. During peak periods — especially the start of a new calendar year when COLA adjustments trigger eligibility questions and January renewals stack up — wait times at busy offices in Baton Rouge, New Orleans, and Shreveport can be significant.
You don't need to visit a regional office to apply. MyMedicaid.la.gov, mail, and fax are equally valid. But if you need face-to-face help completing the form or have questions about required documents, a regional office visit can resolve things faster than phone hold times.
Medicaid Application Centers
Beyond the regional offices, LDH designates Medicaid Application Centers at hospitals, community health centers, Councils on Aging, and other local facilities across the state. These centers are intake and assistance partners — they help you fill out the BHSF Form 1-MB, collect documents, and forward everything to LDH for processing.
Application Centers cannot approve or deny your application. They cannot access your case status in the Medicaid system or tell you whether you've been approved. All formal determinations are made by LDH BHSF eligibility staff at the regional offices.
For many applicants in rural parishes — Tensas, Cameron, Sabine, Concordia — the nearest regional Medicaid office may be a long drive. Application Centers at parish Councils on Aging or local FQHCs offer a closer option for getting help with paperwork. Call your parish Council on Aging to ask whether they serve as an application center and what hours they keep.
Which Office to Contact for Medicare Savings
When calling or visiting about an MSP application specifically, make clear that you're applying for the Medicare Savings Program — QMB, SLMB, or QI — not for full Medicaid, Medicaid long-term care, or Medicaid expansion. The forms are different, the asset rules are different (no asset test for MSP), and the income counting methodology is different (SSI-based, not MAGI).
This matters because general intake staff may default to full Medicaid procedures, which include asset verification and different income thresholds. If an application center starts asking for bank statements for an MSP-only application, remind them that Louisiana eliminated the resource test for QMB, SLMB, and QI under Chart Z-900.

- Online: MyMedicaid.la.gov for the full application, document uploads, and status checks
- Phone: 1-888-342-6207 for help filing or checking status (Monday–Friday, 8:00 AM–4:30 PM Central)
- Mail: P.O. Box 91278, Baton Rouge, LA 70821-9278

An adult child managing the application from out of state can handle everything by mail, fax, or portal — provided the parent has signed BHSF Form AR (Medicaid Authorized Representative Form) to authorize them.
The Louisiana Medicare Savings Programs & Extra Help Guide lists the application channels with exact addresses and includes the authorized representative form instructions for out-of-state caregivers.
