Senior Financial Assistance in Massachusetts: Programs That Pay Medicare Costs
Massachusetts layers three programs that can make Medicare nearly free for seniors on fixed incomes. The combination covers Part B premiums, hospital and doctor cost-sharing, and prescription drugs — but most families do not realize how high the income limits actually go.
Medicare Savings Programs (QMB, SLMB, QI)
These are the foundation. The Medicare Savings Programs pay your parent's Medicare Part B premium ($202.90 per month in 2026), and the top tier — QMB — also eliminates deductibles, coinsurance, and copays.
Massachusetts expanded eligibility in March 2024 by eliminating the asset test entirely and raising income limits to 225% of the federal poverty level. The 2026 income thresholds for a single person:
- QMB: up to $2,527 per month — pays Part B premium, Part A premium (if applicable), $283 annual deductible, and all coinsurance and copays
- SLMB: income above $2,527 up to 210% FPL — pays Part B premium
- QI: income above 210% up to $2,993 per month — pays Part B premium
No bank statements. No retirement account documentation. The only financial question is monthly income.
A single MSP application saves at minimum $2,434.80 per year in Part B premiums alone. With QMB, the savings are substantially higher once deductibles and coinsurance are factored in.
Extra Help (Part D Low-Income Subsidy)
Approval for any MSP tier automatically qualifies your parent for full Extra Help — no separate application required. MassHealth transmits the data electronically to CMS.
For MSP-only beneficiaries, full Extra Help in 2026 covers:
- $0 Part D plan premium for benchmark plans
- $0 Part D deductible (instead of up to $615)
- Copays capped at $5.10 for generic or preferred multi-source drugs and $12.65 for other drugs
- $0 for all covered Part D drugs after reaching the catastrophic threshold
For a senior taking three to five medications, Extra Help can save $1,000 to $3,000 per year on top of the Part B premium savings from MSP.
Prescription Advantage
Massachusetts operates this state pharmacy program through the Executive Office of Aging & Independence. It works as a secondary payer behind Part D, covering copay gaps and specialty drug costs.
The income limit is remarkably high — up to 500% of the federal poverty level, roughly $6,650 per month for a single person. There is no asset test. Enrollment also grants a Special Enrollment Period to switch Part D plans outside the standard annual window.
Prescription Advantage coordinates with Extra Help: if your parent has both, Extra Help pays first and Prescription Advantage covers remaining copay amounts.

How the Programs Stack
Here is what a single person earning $2,400 per month in Social Security gets through these programs:
- MSP (QMB) — MassHealth pays the $202.90 Part B premium, waives the $283 deductible, eliminates doctor and hospital copays. SSA refunds premiums already withheld from Social Security.
- Extra Help — triggered automatically by MSP. For MSP-only beneficiaries, the Part D deductible drops to $0 and 2026 drug copays are capped at $5.10 for generic or preferred multi-source drugs and $12.65 for other drugs.
- Prescription Advantage — secondary copay assistance on top of Extra Help, plus a Special Enrollment Period for plan switching.
Combined annual savings: easily $4,000 to $6,000 depending on medical and prescription utilization.
