Help Paying Medicare Premiums Massachusetts: Every Cost-Relief Option
The 2026 Medicare Part B premium is $202.90 per month — $2,434.80 per year coming straight out of your parent's Social Security check. For someone living on $2,000 a month, that is over ten percent of their gross income. Massachusetts has layered programs that can eliminate that cost entirely, plus cut prescription expenses and even wipe out deductibles and copays.
Medicare Savings Programs: The First Layer
Massachusetts's Medicare Savings Programs (MSPs) are the most valuable benefit most people miss. The state runs three tiers, each paying the Part B premium:
QMB (income up to $2,527/month for individuals): Pays Part A and Part B premiums, the $283 annual Part B deductible, and all Medicare coinsurance and copayments.
SLMB (income between QMB ceiling and 210% FPL): Pays the Part B premium.
QI (income up to $2,993/month for individuals): Pays the Part B premium.
Massachusetts expanded these limits well above federal minimums and eliminated the asset test on March 1, 2024. Savings, retirement accounts, and home equity are completely disregarded. Only income counts.
Extra Help: The Second Layer
Approval for any MSP tier automatically activates full Extra Help (Part D Low-Income Subsidy). Your parent pays $0 for benchmark Part D plan premiums, $0 deductible, and copays capped at $5.10 for generics and $12.65 for brand-name drugs. No separate application is needed — MassHealth sends the data to CMS electronically.
Prescription Advantage: The Third Layer
Massachusetts runs its own state pharmacy assistance program, Prescription Advantage, covering residents with income up to 500% FPL. It works as a secondary payer behind Part D, reducing deductibles and copays further. It also provides a Special Enrollment Period to switch Part D plans outside the annual open enrollment window.
Even seniors who receive full Extra Help through an MSP can benefit from Prescription Advantage's secondary copay assistance and the plan-switching flexibility.

How the Three Programs Stack
For a single Massachusetts senior earning $2,400/month in Social Security:
- MSP (QMB): MassHealth pays the $202.90 Part B premium, the Part B deductible, and all Medicare cost-sharing. Annual savings: $2,434.80 in premiums alone, plus hundreds or thousands in deductibles and copays depending on healthcare usage.
- Extra Help: Automatically activated. Part D premium drops to $0, deductible waived, drug copays capped at a few dollars per fill.
- Prescription Advantage: Secondary copay assistance on anything Extra Help does not fully cover, plus the SPAP Special Enrollment Period.
Total combined annual savings can reach $5,000 to $8,000 or more depending on prescription volume and healthcare utilization.
