How to Apply for the Medicare Savings Program in Indiana

Four Ways to Apply

Indiana's Division of Family Resources (DFR) accepts Medicare Savings Program applications through four channels:

  1. Online — The FSSA Benefits Portal at fssabenefits.in.gov is the fastest route. You complete the eligibility screens, upload verification documents as PDFs, apply an electronic signature, and receive a submission confirmation receipt with a tracking number.

  2. Phone — Call DFR at 1-800-403-0864. A staff member conducts a telephonic interview and records a verbal signature in the IEDSS system.

  3. Mail or fax — Download the state application form, complete it, and mail or fax it to your local county DFR office.

  4. In person — Bring a signed application and supporting documents to any county DFR office.

One important distinction: creating an account on fssabenefits.in.gov does not count as filing an application. The application is legally filed only after you complete all screens, upload documents, sign electronically, and receive the confirmation receipt.

Documents You'll Need

DFR verifies eligibility through electronic data matches and documentary evidence. Have these ready before you start:

  • Medicare card (red, white, and blue card) or proof of Part A entitlement
  • Social Security benefit award letter showing the gross monthly amount
  • Pension or annuity statements (if applicable)
  • Recent bank statements — all pages for every checking and savings account
  • Indiana driver's license or state ID (or utility bill for residency)
  • Burial fund documentation if you want to claim the $1,500 per-person disregard

DFR cross-checks income against SSA and Department of Workforce Development databases, so the gross Social Security amount is what matters — not the net deposit after the Part B premium deduction.

If You're Applying for a Parent

DFR caseworkers cannot discuss a case or accept documents from anyone who isn't the applicant or an authorized representative. To manage a parent's application, you need one of:

  • State Form 55366 (Authorized Representative for Health Coverage) — signed by both you and the parent, granting you authority to complete interviews, submit documents, and receive notices
  • Power of Attorney — if the parent lacks capacity to sign the representative form
  • Letters of Guardianship — court-issued, uploaded to the portal or faxed to DFR

Without one of these on file, DFR generally will not discuss the case with you. If your parent is present during a call, they can give DFR one-time permission to speak with you during that call.

Processing Timeline

Federal regulations give DFR 45 calendar days to process an MSP application. If your application goes past 45 days without a decision:

  1. Locate the 10-digit IEDSS case number from your submission confirmation
  2. Call DFR at 1-800-403-0864 and request a case escalation for overdue processing
  3. If that doesn't resolve it, submit a written inquiry to the DFR Regional Manager for your county

If Documents Are Missing

If DFR needs additional information, they send a Notice of Missing Information with a strict 13-calendar-day deadline. Missing that window results in a technical denial — not because you're ineligible, but because the verification wasn't received in time.

If a renewal is closed for missing documents, Indiana allows a 90-day reinstatement window. Submit the required renewal documents within 90 days of the closure date and DFR reopens the case without requiring a brand-new application.

The Extra Help Shortcut

If you've already applied for Extra Help (the Part D Low-Income Subsidy) through Social Security, SSA may have automatically sent your information to Indiana DFR. Under MIPPA rules, that data transfer counts as a legal MSP application with a protected filing date — and it's exempt from the mandatory interview requirement.
