How to Apply for the Medicare Savings Program in Kentucky

Four Ways to Apply

Kentucky accepts Medicare Savings Program applications through four channels. All four lead to the same DCBS eligibility determination — pick the one that fits your situation.

Online through kynect (kynect.ky.gov): Create a user account, complete the non-MAGI Medicaid section, and upload document scans. This is typically the fastest route for adult children managing a parent's application remotely.

By phone: Call the DCBS Family Support line at 1-855-306-8959 or the kynect support line at 1-855-459-6328. A representative walks through the application over the phone and records an electronic signature.

Paper Form MAP-205: This is the dedicated Medicare Savings Program application form, available at any DCBS county office or downloadable from the CHFS website. Mail it to the Office of the Kentucky Health Benefit Exchange, 12 Mill Creek Park, Frankfort, KY 40601, or return it to your local DCBS office. You can also fax it to the DCBS Centralized Mail Room at 502-573-2005.

In person: Walk into any of Kentucky's 120 county DCBS offices with your documents. Rural applicants in Eastern and Western Kentucky often prefer this route, though travel distances to county seats can be significant.

Documents You'll Need

DCBS uses electronic data matching with SSA, the Kentucky Department of Revenue, and state financial databases to verify most details. You'll only need paper documentation if the automated matches show discrepancies or gaps. Gather these upfront to avoid processing delays:

  • Identity and citizenship: U.S. passport, birth certificate, or driver's license, plus your Social Security card
  • Medicare card: Your red, white, and blue Medicare card showing Part A and Part B effective dates
  • Income proof: Social Security award letter (SSA-1099 or current benefit verification letter), private pension statements, or bank statements showing direct deposits for the last 30 days
  • Asset documentation: Bank statements for all checking, savings, and investment accounts covering 1 to 3 recent months; vehicle titles; life insurance policy cash value statements; any prepaid burial contracts

The Social Security award letter is the single most important document — it shows your gross benefit amount before Part B premiums are deducted, which is the figure DCBS uses for the income calculation.

Applying on Behalf of a Parent

If you're an adult child managing this for your parent, submit Form MAP-14 (Interested Party Authorization) alongside the application. MAP-14 authorizes you to complete applications, submit verifications, receive DCBS notices, and manage the kynect account on your parent's behalf.

Alternatively, a valid Power of Attorney or court-ordered guardianship document serves the same purpose.

What Happens After You Submit

Federal rules require DCBS to issue a decision within 45 calendar days. In practice, Kentucky processing runs 20 to 40 days depending on local office workload and whether your documentation is complete.

One important timing detail: QMB coverage starts the first of the month after approval — it cannot be backdated. SLMB and QI can be retroactive up to three months before the month of application, provided you met eligibility criteria during those months.

After approval, it takes another 60 to 90 days for the Part B premium deduction to stop appearing on your Social Security check. SSA refunds all premiums collected during that lag automatically. Do not contact SSA to cancel Part B during the wait — that would terminate your underlying Medicare coverage.

The Kentucky Medicare Savings Programs & Extra Help Guide includes field-by-field guidance for each application form, the exact income and resource calculations DCBS runs, and a timeline tracker for the full process from application through premium refund.
