How to Apply for Medicare Savings Program in Ohio

Five Ways to Apply

Ohio accepts Medicare Savings Program (MPAP) applications through five channels, all routed to your county Department of Job and Family Services (CDJFS):

1. Online through Ohio Benefits. Go to benefits.ohio.gov, create or log in with your OHID account, and submit the application electronically. You can upload verification documents directly. The portal is available 24/7, though system timeouts and login issues are common enough that you should save your progress frequently.

2. In person at your county JFS office. Walk into the CDJFS office in the applicant's county of residence. Bring your documents — you may be able to meet with an eligibility worker that day, or they'll schedule an appointment.

3. By phone. Call the Ohio Medicaid Consumer Hotline at 1-800-324-8680 (TTY: 1-800-292-3572) or your county JFS intake line. A worker can take the application over the phone.

4. By mail or fax. Complete Form ODM 07216 (Application for Health Coverage & Help Paying Costs), sign it, and mail or fax it to your county JFS office. The county's processing period runs from the date it receives the application, so keep proof of mailing or fax transmission.

5. Through an SSA Extra Help application. Filing Form SSA-1020 for Part D Extra Help at your local Social Security office triggers an automatic data transfer to Ohio Medicaid under the MIPPA law. This counts as a protected filing date for MPAP — but the data transfer can lag 60 to 90 days, so filing directly with the county is faster if premium relief is urgent.

Documents You'll Need

Gather these before applying to avoid verification delays:

  • Medicare card (red, white, and blue) or SSA award letter confirming Part A and Part B enrollment
  • Ohio residency proof — driver's license, state ID, utility bill, or lease
  • Income documentation — Social Security benefit statement (SSA-1099 or current award letter), pension statements, pay stubs from the last 30 days, or VA award letter
  • Resource documentation — bank statements for all accounts covering the most recent full month, burial contract documentation, life insurance cash surrender value statements

County caseworkers check electronic data sources (SSA feeds, wage databases, asset verification systems) before asking you for additional paperwork, so you may not need everything. But having it ready prevents the back-and-forth that stretches processing time.

Processing Timeline

CDJFS must process your application within 45 calendar days of receipt. If disability needs to be established, the deadline extends to 90 calendar days. Once approved, the caseworker enters the approval into Ohio Benefits, which sends a buy-in record to CMS. The Social Security Administration then stops deducting the Part B premium from the beneficiary's check — but this inter-agency pipeline takes 60 to 90 days to complete.

During that gap, Part B premiums continue being withheld. When the buy-in finally processes, SSA issues a retroactive lump-sum refund for all the premiums deducted after the effective date. Don't cancel Part B enrollment while waiting — that would terminate Medicare coverage entirely.

Applying for a Parent as an Out-of-State Caregiver

You can handle most of the process remotely. File online through Ohio Benefits, mail the paper application, or call the county JFS. To manage the case on your parent's behalf, have them sign the authorized representative section (Section 11 of Form ODM 07216) or submit a separate Form ODM 08065.

Your parent remains the legal applicant — your income and assets are never counted in their eligibility determination, even if they live with you.
