How to Apply for Medicare Savings Program in Missouri

Which Form to Use

If you're applying for MSP coverage only (QMB, SLMB, or QI-1 without full Medicaid), submit Form IM-1MSP — the streamlined Medicare Savings Programs application. If you're applying for full MO HealthNet alongside MSP, you'll need the IM-1ABDS (Aged, Blind, and Disabled Supplement) as well.

Both forms are available on the Missouri DSS website and at any FSD Resource Center.

Four Ways to Submit

Online through myDSS (mydss.mo.gov): Create an account, fill out the application electronically, and upload supporting documents through the DSS Upload Portal at mydssupload.mo.gov. You can track your case status through the same portal after submission.

By phone (855-373-4636): An FSD representative takes your application over the phone. You'll still need to submit financial documents separately by fax, mail, or upload.

By fax (855-600-3754): Fax the completed IM-1MSP and all supporting documents to the FSD Centralized Processing Center. Keep the fax confirmation sheet — it's your proof of the filing date.

In person: Walk into any FSD Resource Center. Staff can help you complete the form and accept your documents on the spot. Expect wait times, especially at St. Louis and Kansas City metro offices.

Documents You'll Need

Category What FSD Accepts
Identity Driver's license, state ID, passport, or birth certificate
Citizenship U.S. passport, naturalization certificate, green card, or SSA data match
Missouri residency Utility bill, lease, property tax receipt, or voter registration card
Income SSA-1099 or award letter, pension statements, VA benefit letter, recent paystubs
Assets Complete bank statements for all accounts (every page)
Life insurance Policy document showing face value and cash surrender value
Vehicles Title, registration, or loan balance for vehicles beyond your primary car
Burial provisions Prepaid burial contracts, trust documents, or dedicated account records

Submit all pages of bank statements. Missing pages can delay processing, so include every page.

If You're Applying for a Parent

File Form IM-6AR (Appointing an Authorized Representative) alongside the application. This authorizes FSD to give you portal access, send you copies of notices, and discuss the case with you directly. A regular power of attorney is not enough — FSD requires its own authorization form before it will talk to you about someone else's case.

Also consider filing Form IM-6 (Authorization for Release of Information), which lets FSD pull records directly from banks and medical providers. This can speed up verification.

Processing Timeline

Federal rules under 42 CFR 435.912 require states to process non-MAGI Medicaid applications within 45 days. In practice, complex cases or missing documents can push this timeline out. You can check status through myDSS, the chat interface on the portal, or by calling 855-373-4636.

If you're also filing for Extra Help (Part D Low-Income Subsidy) through SSA, the date SSA receives that application serves as a protective filing date for your Missouri MSP application under the MIPPA Act. FSD still verifies assets and Missouri residency before deciding eligibility, and the filing date does not guarantee approval or determine your coverage start date.

When Coverage Kicks In

This depends on the program tier:

  • QMB: First day of the month after FSD approves — no retroactive coverage
  • SLMB: Potentially retroactive up to three calendar months before the application month, if you met eligibility during those months
  • QI-1: Same as SLMB — up to three calendar months retroactive

File as early as possible, especially for QMB, which has no retroactive coverage. A delay can mean more premiums and cost-sharing paid out of pocket.
