How to Apply for Medicare Savings Program in Idaho
Five Ways to Apply
Idaho's Department of Health and Welfare accepts Medicare Savings Program applications through five channels. You don't need to pick the "right" one — they all feed into the same statewide processing queue, and a caseworker in any regional office can handle the application regardless of where your parent lives.
Online: Create an account at idalink.idaho.gov and submit the integrated health coverage application. This is the fastest route — the system confirms receipt immediately.
Paper: Complete Form HW2019 (Application for Health Coverage) and mail it to your parent's regional DHW office or the centralized address.
Fax: Send the completed HW2019 to the centralized intake fax at 1-866-434-8278. Keep the fax confirmation page as proof of filing.
Phone: Call DHW at 1-877-456-1233 and apply verbally. A caseworker walks through the questions.
In person: Visit any DHW Self-Reliance field office — major locations include Boise, Nampa, Idaho Falls, Pocatello, Twin Falls, Lewiston, and Coeur d'Alene.
The Form: HW2019
Form HW2019 is the unified application Idaho uses for Medicaid, MSPs, SNAP, and other programs. You don't file a separate MSP-specific application. When completing HW2019, the medical assistance section is where MSP eligibility gets evaluated.
If you're helping a parent apply, you'll also need to complete the Authorized Representative section of HW2019 — this gives you legal authority to discuss the case with DHW. Without it, caseworkers cannot share case details with you, even if you submitted the application.
Documents to Gather Before You Start
DHW needs to verify identity, income, and resources. Missing documents are the single most common cause of processing delays. Collect these before submitting anything:
- Medicare card showing the Medicare Beneficiary Identifier (MBI) and Part A/B coverage dates
- Idaho ID or driver's license (or utility bill showing physical address for residency proof)
- Social Security award letter showing the gross monthly benefit — not the bank deposit, which reflects deductions
- Pension or annuity statements if your parent receives any
- Pay stubs for the last 30 days if your parent works part-time
- Bank statements for every checking and savings account, covering the most recent full month — DHW looks at account numbers, institution names, and current balances
- Vehicle titles for any cars beyond the primary vehicle (one car is exempt)
- Life insurance policy schedules showing face value and cash surrender value
- Marriage certificate and spousal income/asset documentation if your parent is married

Processing Timeline
The maximum processing standard is 45 days for a non-disability MSP application under federal regulations. Applications requiring a disability determination for applicants under 65 can take up to 90 days.
If the application hasn't been decided after 45 days and the delay isn't caused by missing documents on your end, call 1-877-456-1233 and ask for a supervisor review, or submit a written inquiry to the regional office.
What Happens After Submission
DHW reviews the application and may send a verification request letter if anything is missing or unclear. Respond by the deadline printed on the letter or the application can be denied procedurally.
Once approved, DHW sends an electronic file to CMS and SSA. The Part B premium deduction from Social Security stops within 30 to 90 days. For SLMB and QI, SSA issues a retroactive reimbursement check for premiums already paid during the processing period (up to three calendar months before the month of application, if the applicant met the criteria in those months). QMB coverage starts prospectively — the first of the month after the approval decision.
Common Application Mistakes
Reporting net instead of gross income. DHW uses gross Social Security — the full entitlement amount before Part B premiums, tax withholdings, or garnishments. The gross figure appears on the annual Social Security award letter or at ssa.gov.
Forgetting the Authorized Representative form. If an adult child submits the application, the representative section of HW2019 must be completed and signed. Without it, DHW cannot discuss the case with you.
Wrong program category in idalink. The idalink portal combines applications for SNAP, MAGI Medicaid, and MSPs. Selecting the wrong category can trigger requests for unnecessary work verification documents.
The Idaho Medicare Savings Programs & Extra Help Guide includes a pre-submission document checklist organized by what DHW actually asks for, plus an application submission tracker for logging confirmation numbers, dates, and follow-up deadlines.
