How to Apply for the Medicare Savings Program in Utah
Applying for a Medicare Savings Program in Utah goes through the Department of Workforce Services — not Medicare, not Social Security. Here's how each application route works and what you'll need to have ready.
Four Ways to Apply
1. Online through myCase (fastest) Go to jobs.utah.gov/mycase and log in with a UtahID account. Select "Apply for Benefits," check "Medical Assistance," and work through the eligibility questions. You can upload verification documents directly and track your application status in real time.
If you're filing on behalf of a parent, you can link your UtahID to their case after submitting Form 114AR (Authorization to Disclose Medical Eligibility Information).
2. By phone Call the DWS Eligibility Customer Service line at 1-866-435-7414. A worker will collect the information over the phone and route document requests.
3. On paper (Form 61) Download and complete Form 61 — the Application for SNAP, Financial Assistance, Child Care, and Medical Assistance. It covers all DWS programs in one form, which is why it runs 20+ pages. You only need to complete the sections relevant to medical assistance.
Mail it to: DWS Medical Processing Office, P.O. Box 143245, Salt Lake City, UT 84114-3245. Or fax to (801) 526-9505.
4. In person Walk into any DWS Employment Center across Utah. You can submit your application and paper documents directly. Staff can also help you access a public computer to apply through myCase.
Documents You'll Need
DWS verifies much of this electronically through SSA databases, state wage records, and the Asset Verification System. But they may request paper copies for anything they can't confirm automatically. Have these ready:
- Government-issued photo ID
- Medicare card (the red-white-and-blue card) or SSA award letter confirming Part A entitlement
- Most recent Social Security Award letter showing gross monthly benefit
- Pension or annuity statements
- Bank statements for all accounts covering the prior 30–60 days
- Proof of Utah residency (utility bill, lease agreement)
If DWS needs additional documentation they can't find electronically, they'll send a written Request for Information with a 10–15 day deadline.
You Don't Pick Your Program
You don't need to specify whether you want QMB, SLMB, or QI. DWS evaluates your income and resources against all three program levels and assigns the most beneficial one you qualify for.

Processing Timeline
Federal rules require DWS to issue a final decision within 45 calendar days from the date they receive a signed application. If eligibility depends on a newly claimed disability, the window extends to 90 days.
If Someone Else Is Applying for Your Parent
An adult child or other representative needs Form 114AR on file with DWS. This form authorizes you to speak with caseworkers, receive official notices, submit documents, and file appeals on your parent's behalf. For Social Security matters (like Extra Help), you'd need a separate SSA Form SSA-1696 filed at the local SSA office.
The Extra Help Shortcut
If your parent files an Extra Help application directly with Social Security, SSA automatically transmits that data to DWS. Utah treats it as a formal MSP application, using the SSA filing date as the application date. DWS may follow up for supplemental verification, but your parent won't need to file a separate MSP application.
The Utah Medicare Savings Programs & Extra Help Guide walks through the entire application process section by section, including how to navigate the myCase portal and which Form 61 fields to focus on for MSP-only applicants.
