$0 Vermont — Medicare Savings Program Eligibility Checklist

How to Apply for the Medicare Savings Program in Vermont

Vermont's Medicare Savings Programs can save your parent over $2,400 a year in Part B premiums alone — but the application process runs through a tangle of agencies, forms, and processing centers that trips up most families on the first try.

Pick the Right Form

This is where most applications stall. Vermont uses different forms depending on what your parent needs:

Form 201P is the streamlined application for Medicare Savings Programs (QMB, QI) and pharmacy assistance (VPharm). It focuses on income and household composition. No detailed asset questions. Use this form if your parent only needs Medicare premium help and prescription assistance.

Form 205ALLMED is the comprehensive application for full Medicaid for the Aged, Blind, and Disabled (MABD) plus MSP. It includes detailed resource and asset sections. Use this form only if your parent also needs full Medicaid coverage (dental, vision, long-term care).

Filing 205ALLMED when 201P would suffice doesn't help — it adds processing complexity and asks for asset documentation your parent doesn't need to provide for MSP.

Four Ways to Submit

Online: Upload the completed form through the Vermont Document Uploader at my.vermont.gov. Select "healthcare" as the program category. You can upload scanned or photographed pages of the signed form and supporting documents.

Mail: Send the packet to Green Mountain Care Application and Document Processing Center, 280 State Drive, NOB 1 South, Waterbury, VT 05671-1500.

Phone: Call Green Mountain Care Customer Service at 1-800-250-8427, Monday through Friday 8:00 AM to 4:30 PM, and complete Form 201P verbally over the phone.

In person: Visit any DCF Economic Services Division district office or schedule an appointment with an Area Agency on Aging assister.

Documents to Gather Before You Start

The application needs income verification. Collect these before sitting down with the form:

  • Social Security award letter (the most recent one showing gross monthly benefit amount)
  • Pension statements showing gross monthly payments
  • Last 30 days of pay stubs if your parent still works
  • Most recent federal tax return with all schedules if self-employed

Vermont's automated data matching with SSA and the state Department of Taxes can verify many items electronically. If the electronic match confirms what your parent reported, no additional paper verification is required.

The 45-Day Processing Clock

DVHA must issue an eligibility decision within 45 days of receiving a completed application. If eligibility depends on a disability determination for someone under 65 without established SSA disability status, the window extends to 90 days.

DVHA's 45-day period begins when it receives a completed application. An unsigned form or missing required verification can mean the application is not yet complete.

What Happens After Approval

QMB coverage starts on the first day of the calendar month after the month in which DVHA issues the final approval notice. It is strictly prospective — no retroactive coverage. QI can be retroactive up to three months before the application month, as long as your parent was eligible during those months and the retroactive period falls within the same calendar year.

After approval, Social Security may continue deducting the Part B premium from your parent's check for one to three months while the state buy-in processes through federal systems. Those premiums are automatically refunded once SSA completes the accretion.

The Vermont Medicare Savings Programs, Extra Help & VPharm Guide covers the complete submission process, field-by-field form guidance, and a timeline of what to expect from approval through premium refund.
