How to Apply for Iowa Medicare Savings Program
The application for Iowa's Medicare Savings Programs is simpler than most families expect — one form covers QMB, SLMB, and E-SLMB/QI, and Iowa HHS determines which tier fits based on the numbers. The hard part isn't the form itself. It's gathering the right documents and understanding the timeline so nothing falls through the cracks.
Four Ways to Apply
Online — the fastest option. The Iowa HHS Services Portal at hhsservices.iowa.gov lets you file electronically under "Medicaid and Health Coverage." Creating an account (rather than filing as a guest) lets you save progress, upload verification documents, and track application status in real time.
Phone — call Iowa HHS at 1-855-889-7985 (TTY: 7-1-1). A caseworker completes the application over the phone and captures a telephonic signature. Good for applicants without internet access or who need help navigating the questions.
Paper — Form 470-5170 ("Application for Health Coverage and Help Paying Costs") is the standard paper application. A Spanish version (470-5170(S)) is also available. Submit by mail, fax, or hand-deliver to any Iowa HHS county service center.
In person — walk into any Iowa HHS county office. Staff can assist with filling out the application and scan documents on the spot.
You don't need to specify which MSP tier you want. Iowa HHS evaluates the applicant's net countable income and assets, then assigns QMB, SLMB, or E-SLMB/QI accordingly.
Documents to Have Ready
Iowa HHS runs automated checks through federal databases and state records to verify identity and income. If those electronic checks confirm everything, you may not need to submit any paper documentation. But automated verification doesn't always find everything, so have these ready:
- Social Security Award Letter — the most recent one, showing gross monthly benefit amount (before Part B deductions)
- Bank statements — checking, savings, and investment account statements from the most recent month
- Proof of Iowa residency — driver's license, state ID, utility bill, or lease agreement
- Proof of citizenship or immigration status — birth certificate, U.S. passport, or qualifying immigration documents
- Pension or annuity statements — if the applicant receives private pension income or VA benefits
- Medicare card — showing Part A and/or Part B enrollment
If Iowa HHS needs additional verification after submission, they mail a request specifying exactly which documents are missing and a deadline to respond.
The Form 470-4846 Trap
Here's where many applications silently fail. When someone applies for Extra Help through Social Security, SSA automatically shares that data with Iowa HHS — unless the applicant opts out. Iowa HHS then mails Form 470-4846 ("Medicare Savings Programs Additional Information Request"), pre-populated with SSA data.
The form arrives with a strict return deadline. If it isn't completed, signed, and returned with any required verifications within that window, Iowa HHS's ELIAS system automatically denies the application on day 30. Many families mistake this form for junk mail or assume the SSA filing handled everything.
If you filed for Extra Help through SSA, watch the mail for Form 470-4846 from Iowa HHS. It's not optional.

Applying as an Authorized Representative
Adult children helping a parent can be designated as an authorized representative directly on Form 470-5170 — there's a dedicated section for it. The parent signs the form giving the representative authority to complete filings, receive correspondence, and manage the case.
For appeals, a separate form is required: Form 470-5526 ("Authorized Representative for Appeals"). Both the parent and the representative sign it.
A Durable Power of Attorney or court-issued guardianship order also works in place of the HHS-specific forms. But note: designating an authorized representative with Iowa HHS doesn't automatically extend to Social Security or Medicare — those agencies have their own representative processes.
After You Submit
Iowa HHS has 45 calendar days to process the application (90 days if a disability determination is needed). The decision arrives as a written Notice of Decision specifying approval or denial, the assigned MSP tier, calculated income, and appeal rights.
Coverage start dates depend on the tier. QMB starts on the first of the month after approval and cannot be backdated. SLMB and E-SLMB can be made retroactive up to three months before the application month, provided the applicant was eligible during those months. That retroactive coverage means Part B premiums withheld during those months get refunded by Social Security.
The Iowa Medicare Savings Programs & Extra Help Guide includes a document preparation checklist and a post-submission tracking log for monitoring the 45-day processing window and premium refund timeline.
