How to Apply for Medicare Savings Program in Nebraska
The biggest reason Nebraska MSP applications stall isn't ineligibility — it's incomplete paperwork. DHHS sends a verification request, the family doesn't respond within 10 to 14 days, and the case gets closed. Having every document ready before you submit is the difference between a 30-day approval and a four-month cycle of rejections and resubmissions.
Four Ways to Apply
iServe Nebraska (online) is the primary intake channel. Create an account at iserve.nebraska.gov, select "Apply for Benefits," and navigate to the Medical Assistance section. The portal lets you upload verification documents directly. This replaced the older ACCESSNebraska interface.
Paper application uses Form MLTC-64 (Application for Nebraska Medicaid for Aged and Disabled). Mail it to the Nebraska DHHS Document Imaging Center at P.O. Box 2992, Omaha, NE 68103-2992, or fax it to (402) 742-2351.
Phone application — call DHHS Medicaid Customer Service at (855) 632-7633 or (402) 473-7000 to start an application verbally.
In-person at any Nebraska DHHS public assistance office. Offices in Omaha, Lincoln, Grand Island, and Scottsbluff have self-service kiosks.
Documents to Gather Before You Start
Get these together before you open iServe or print the form:
- Identity: Driver's license, passport, or birth certificate paired with Social Security card
- Medicare card: The red, white, and blue card showing Part A and Part B enrollment, or an SSA entitlement notice with your MBI number
- Income proof: SSA benefit verification letter (the annual one or request a current one at ssa.gov), pension pay stubs, annuity statements — anything showing gross monthly income for the most recent 30 days
- Bank statements: Complete statements for every checking, savings, and investment account for the full calendar month before the application date
- Life insurance: Face value documentation for any whole life policies (term life doesn't count against resources)
If an adult child is applying on behalf of a parent, submit Form MLTC-35 (Designation of Authorized Representative) or use the representative authorization option in iServe. Without authorization on file, DHHS cannot share case information with you.
Processing Timeline
DHHS must process applications within 45 calendar days. In practice, most straightforward cases close in 20 to 35 days when documentation is complete.
Coverage start dates vary by tier:
- QMB: Prospective only — coverage starts the first day of the month after approval. No retroactive coverage.
- SLMB: Retroactive up to three months before the application month, if you were eligible during those months.
- QI: Retroactive up to three months, but it cannot cross into the previous calendar year.

What Happens If the Application Stalls
If 45 days pass without a decision or a formal delay notice, call (855) 632-7633 and ask for a supervisor review. You can also file a fair hearing request based on agency inaction under Title 477 NAC Chapter 10.
The most common stall is a verification request you missed. Check iServe's Action Alerts regularly after submitting — DHHS posts document requests there, and the clock on your response is short.
The Nebraska Medicare Savings Programs & Extra Help Guide walks through the entire application process with a pre-submission document checklist and a tracking log for monitoring your case from submission through SSA premium buyback.
