W-1QMB Application Connecticut: How to Fill Out the MSP Form
W-1QMB vs W-1E: Use the Right Form
Connecticut DSS offers two forms that can be used for Medicare Savings Program applications, and using the wrong one creates unnecessary work:
Form W-1QMB is the dedicated MSP application. It's two pages long and asks only for the information DSS needs: personal details, Medicare status, and income. No asset questions, no bank account disclosures, no property valuations.
Form W-1E is the general benefits application for SNAP, cash assistance, HUSKY C Medicaid, and MSP. It runs 15+ pages and demands detailed asset documentation — bank statements, savings accounts, property ownership — that MSP doesn't require. Connecticut eliminated the MSP asset test under CGS § 17b-95, so every asset question on W-1E is irrelevant for MSP-only applicants.
State search results and even some DSS office staff default to W-1E. If you're applying only for the Medicare Savings Program, insist on W-1QMB.
What the W-1QMB Asks For
The form covers four areas:
Personal information — Name, address, date of birth, Social Security number, citizenship or immigration status, Connecticut residency.
Medicare information — Medicare Beneficiary Identifier (MBI) from the Medicare card, Part A and Part B enrollment dates.
Income — Gross monthly income from all sources: Social Security, pensions, wages, self-employment, interest, dividends, rental income. DSS wants the gross amount before any deductions. For Social Security, use the gross benefit amount (before Part B premium deductions), which appears on the annual award letter, not the deposited amount.
Spouse information — If married and living together, the spouse's income must be reported even if only one person is applying.
Documents to Attach
Attach copies (not originals) of:
- Medicare card showing MBI, or SSA Award Letter confirming Part A and Part B enrollment
- Most recent Social Security award letter showing gross monthly benefit
- Pay stubs covering four consecutive weeks if currently working
- Pension or annuity distribution statements
- Driver's license, state ID, or utility bill for identity and residency
Do not attach bank statements, savings account records, or any asset documentation. If you're applying only for MSP, those documents are unnecessary and can slow processing.
Include a FastLink Cover Sheet with your mailing — it helps the scanning center route the application correctly.

Where to Submit
By mail: DSS ConneCT Scanning Center, PO Box 1320, Manchester, CT 06045-1320
Online: Through the ConneCT portal at connect.ct.gov. Upload documents directly.
In person: Any DSS Resource Center (Hartford, New Haven, Bridgeport, Waterbury, Norwich). Staff handle identity verification on the spot, though processing still goes through the central system.
By fax: To your regional DSS Resource Center fax number.
Common Mistakes
Reporting net Social Security instead of gross. DSS needs the gross amount before Part B premium deductions. The amount deposited into a bank account is net — it's already reduced by $202.90. Use the SSA award letter for the correct gross figure.
Submitting W-1E instead of W-1QMB. The longer form creates processing delays and may trigger improper requests for asset documentation.
Forgetting Form W-298 for authorized representatives. If an adult child is handling the application for a parent, DSS requires its own authorization form. Generic healthcare POAs or SSA-issued powers of attorney are often rejected without a state-specific release.
After Submission
DSS must act within 45 days. If the application is approved, DSS assigns the appropriate tier (QMB, SLMB, or ALMB) based on reported income and issues a Notice of Action. The state then begins the buy-in process with CMS and SSA to stop Part B premium deductions — that takes an additional 60 to 90 days.
