Where to Mail Connecticut MSP Application — DSS Scanning Center and Documents
Connecticut does not have county welfare offices — the state abolished county government in 1960. All paper MSP applications go to a single centralized intake address. Getting the address right and including the correct documents is the difference between a 45-day decision and a months-long delay.
The Mailing Address
Every paper application, renewal form, and verification document for the Medicare Savings Program goes to:
DSS ConneCT Scanning Center P.O. Box 1320 Manchester, CT 06045-1320
This is the centralized document processing hub for the entire state. Mail, faxed documents, and in-person submissions at regional DSS Resource Centers all get routed here for electronic scanning into the statewide ImpaCT eligibility database.
Always Include the FastLink Cover Sheet
Attach a DSS FastLink Cover Sheet to every paper submission. The cover sheet has a barcode that ties your documents to the correct case in the ImpaCT system. Without it, your application can get delayed in manual processing queues while workers try to match documents to the right applicant.
The cover sheet is available from the DSS website, from CHOICES counselors, and from any regional DSS Resource Center.
Documents You Need to Include
Because Connecticut eliminated the asset test for MSP (CGS § 17b-95), the document list is shorter than you might expect. DSS cannot legally ask for bank statements, savings account records, real estate deeds, or any asset documentation for an MSP-only application.
What DSS does need:
Medicare verification — a copy of the Medicare card (the red, white, and blue card) showing the Medicare Beneficiary Identifier, or an SSA award letter confirming Part A and Part B entitlement.
Income verification — gross Social Security award letter (the most recent annual COLA notice works), pay stubs covering four consecutive weeks if anyone in the household works, pension statements, or annuity distribution forms.
Residency and identity — a Connecticut driver's license, state ID, or utility bill showing a Connecticut address.

If DSS Asks for Asset Documents
If you receive a Form W-1348 (Verification Request List) demanding bank statements or asset documentation for an MSP-only application, escalate immediately. Cite CGS § 17b-95 and request a supervisor review. DSS eligibility workers are legally prohibited from applying asset tests to MSP applications. This comes up more than it should — the same workers process HUSKY C Medicaid applications, which do have asset limits, and the wrong form gets sent.
Alternative Submission Methods
Online — the ConneCT portal (connect.ct.gov) allows digital application submission and document upload. The myDSS mobile app offers the same functionality. Both provide immediate upload confirmation.
Fax — documents can be faxed to your regional DSS Resource Center. Call first to confirm the current fax number.
In person — regional DSS Resource Centers in Hartford, New Haven, Bridgeport, Waterbury, and Norwich accept walk-in submissions. Staff can help with identity verification on the spot. Processing still happens through the centralized Manchester system.
Tracking Your Submission
After mailing your application, check status through the ConneCT portal or call the DSS Benefits Center at 1-855-626-6632. DSS is bound by a 45-day processing standard from receipt to decision. If your case passes 45 days without a determination, contact a CHOICES counselor (1-800-994-9422) to escalate, or file a fair hearing request for agency delay.
The Connecticut Medicare Savings Programs & Extra Help Guide includes a complete document checklist and a submission tracker so you can log the mailing date, confirmation steps, and processing milestones from application through approval.
