How to Apply for QMB in Maryland: myMDTHINK, Forms, and Documents

Three Ways to File a Maryland MSP Application

Maryland accepts Medicare Savings Program applications through three channels. All three lead to the same local Department of Social Services office for processing.

Online through myMDTHINK — File at mymdthink.maryland.gov. This is the state's primary electronic portal for public assistance applications, including QMB, SLMB, and QI. You create an account, fill out the application, and upload supporting documents.

One important distinction: do not use Maryland Health Connection for this. Maryland Health Connection handles MAGI Medicaid and marketplace health plans. MSP applications for Medicare beneficiaries go through myMDTHINK. Filing on the wrong portal creates routing delays.

Paper application by mail — Complete DHS/FIA Form 9705 (QMB/SLMB/MSP Application) and mail or drop it off at your local DSS office. Get a stamped receipt if you drop it off in person — paper applications can get lost, and the receipt establishes your filing date.

By phone — Call the DHS Consolidated Services Center at 1-800-332-6347 to initiate an application. A representative walks you through the intake over the phone.

Documents You'll Need

Have these ready before you start, regardless of which channel you use:

  • Proof of identity and Maryland residency — driver's license or state ID, plus a utility bill or lease
  • Medicare card — showing Part A and/or Part B effective dates
  • Income verification — Social Security award letter, pension statements, VA benefit letters, or 30 days of pay stubs for earned income
  • Bank statements — most recent statements for all checking, savings, and investment accounts (all pages)
  • Marital status — marriage certificate, divorce decree, or death certificate if widowed

Maryland counts resources as of the first day of the month, so your statements should cover that date.

If You're Applying for a Parent

Being an adult child doesn't automatically let you access your parent's case file or speak with their caseworker. Maryland requires formal authorization before a family member can handle an MSP application on someone else's behalf.

Your options:

  • Authorized Representative section on Form DHS/FIA 9705 — your parent signs this to designate you as their representative during the application
  • Financial Power of Attorney — must explicitly cover public benefits or healthcare administration
  • Court-ordered guardianship — if your parent cannot sign for themselves

Complete the authorization section with the application. Without it on file, the caseworker cannot tell you whether documents were received, what's missing, or when a decision was made.

What Happens After You Apply

Federal rules give Maryland 45 calendar days to process an MSP application. If the local DSS doesn't issue a decision within 45 days, you have an immediate right to request a fair hearing to force adjudication.

After approval, it takes another 60 to 90 days for the system to fully update across Maryland's Medicaid agency, CMS, and Social Security. During that lag, the Part B premium may still come out of the Social Security check. Once SSA processes the state buy-in, it issues a retroactive lump-sum refund for every premium withheld since the MSP effective date.

SLMB and QI coverage can be backdated up to three months before the application month. QMB coverage starts the first of the month after the approval decision — no retroactive coverage.

The Maryland Medicare Savings Programs, Extra Help & SPDAP Guide includes a document checklist, a premium refund tracker, and an agency communication log for keeping your application on track.
