How to Apply for QMB in DC — District Direct, ESA Centers, and What Documents You Need

Applying for DC's Medicare Savings Program is straightforward once you know which agency handles it and what paperwork they need. The process goes through the Department of Human Services Economic Security Administration (ESA) — not Medicare, not Social Security, and not DC Health Link.

Here's every path to get the application submitted.

Four Ways to Apply

Online — District Direct (districtdirect.dc.gov) The fastest option. Create an account, select "Apply for Benefits," and complete the Combined Application for Benefits. Check the box for "Medical Assistance / QMB." Upload supporting documents directly through the portal.

By phone — (202) 727-5355 The ESA Public Benefits Call Center can take an application over the phone. They'll walk through the same form and tell you what documents to submit afterward.

In person — any ESA Service Center Walk in Monday through Friday, 7:30 AM to 4:45 PM. Bring your documents. Five locations are open:

Service Center Address Phone
Anacostia 2100 Martin Luther King Jr. Ave SE (202) 645-4614
Congress Heights 4001 South Capitol St SW (202) 645-4525
Fort Davis 3851 Alabama Ave SE (202) 645-4500
H Street 645 H St NE (202) 698-4350
Taylor Street 1207 Taylor St NW (202) 576-8000

By mail or fax Mail the completed Form v27 to: Case Record Management Unit, P.O. Box 91560, Washington, DC 20090. Or fax to (202) 671-4400.

Documents You Need

ESA requires verification in four areas:

  1. Medicare enrollment — your parent's red, white, and blue Medicare card, or an SSA Benefit Award Letter
  2. Identity — driver's license, DC non-driver ID, or passport
  3. DC residency — utility bill, lease agreement, or rent receipt
  4. Income — SSA-1099 (annual Social Security statement), pension stubs, or 3 months of bank statements showing direct deposit amounts

Don't send Social Security numbers via unencrypted email. Use District Direct's secure upload, certified mail, or drop the documents in person.

The Form v27 Walkthrough

The Combined Application for Benefits covers multiple programs at once — SNAP, Cash Assistance, and Medicaid. When your parent fills it out for QMB:

  • Check the "Medical Assistance / QMB" box in the program selection section
  • Answer all income questions using gross (pre-deduction) amounts
  • The resource/asset questions on the form cover other programs — for QMB, ESA ignores those answers entirely (DC has no asset test for its Medicare Savings Program)
  • If your parent wants to apply for SNAP or other programs simultaneously, fill out the entire form

If You're Applying on Behalf of a Parent

An adult child can file the application for their parent. Two requirements:

  1. Your parent signs an Authorized Representative Designation Form giving you permission to discuss the case with ESA
  2. If using District Direct, create the representative account under your own identity and link it to your parent's case — don't create an account impersonating your parent

A power of attorney provides broad legal authority but isn't automatically recognized by ESA. You'll still need to upload the POA documentation to District Direct or submit the ESA authorization form.

What Happens After You Apply

ESA has 45 days to issue a decision for aged applicants, or 90 days if a disability determination is required. During that window:

  • ESA may send an Information Request Notice asking for missing documents (you'll typically get 10–15 days to respond)
  • An approval notice will state the program category, effective start date, and confirm $0 cost-sharing liability
  • A denial notice will cite the specific legal ground and explain how to appeal through the Office of Administrative Hearings

Coverage starts on the first day of the month after the approval month. If ESA approves on March 15, QMB takes effect April 1. Coverage cannot be made retroactive.
