Texas HICAP — Free Medicare Counseling and What It Can't Do for You

When you call the Texas Health Information, Counseling and Advocacy Program (HICAP) at 1-800-252-9240, you reach a certified volunteer counselor trained to explain Medicare options without selling you anything. It's genuinely useful — and genuinely limited. Knowing both sides helps you decide whether HICAP is enough or whether your family needs more.

What HICAP Actually Does

HICAP is Texas's version of the national State Health Insurance Assistance Program (SHIP). It operates through a partnership between HHSC, the Texas Legal Services Center, and 28 regional Area Agencies on Aging across the state.

Counselors can help with:

  • Medicare plan comparisons — Original Medicare vs. Medicare Advantage, and Part D plan options
  • Basic MSP screening — whether your parent's income and assets might qualify for QMB, SLMB, or QI
  • Extra Help eligibility — explaining Part D Low-Income Subsidy and how to apply through SSA
  • Medicare enrollment periods — when your parent can sign up or switch plans
  • Rights and protections — explaining QMB billing protections in general terms

HICAP counselors are unbiased. They don't sell insurance, don't take commissions, and don't steer people toward specific Medicare Advantage plans. That alone makes them more trustworthy than most of the "Medicare help" websites that show up in search results.

Where HICAP Stops

HICAP counselors are advisors, not case managers. They can explain how a program works, but they can't manage your parent's application for them. Specifically:

  • They won't fill out Form H1200-EZ line by line for your parent
  • They won't organize financial documents like bank statements and SSA-1099s into a submission-ready packet
  • They won't draft balance billing dispute letters if a provider illegally bills your parent after QMB approval
  • They won't track the SSA reimbursement to make sure the Part B premium deduction actually stops
  • They won't monitor renewal deadlines and remind your family when the recertification packet is due

During peak enrollment periods — especially October through December for Medicare open enrollment — appointment wait times at local Area Agencies on Aging can stretch to several weeks. Rural counties may not have a local counselor at all.

Other Free Resources

Beyond HICAP, Texas has several other channels:

  • 2-1-1 Texas (Option 2 for MEPD) — HHSC's intake line for benefits questions and phone applications
  • Area Agencies on Aging — local offices that can connect families with benefits counseling and caregiver support
  • Aging and Disability Resource Centers (ADRCs) — call 855-937-2372 for community referrals
  • Texas Legal Services Center (1-800-252-3342) — free legal help for Medicare appeals and benefit disputes

Each of these covers a piece of the puzzle. None of them provides a single, consolidated walkthrough of the entire MSP application-to-approval-to-renewal process.

When a Guide Complements Free Counseling

HICAP is the right first call for "does my parent qualify?" A process guide fills the gap between that initial screening and actually getting benefits running. The Texas Medicare Savings Programs & Extra Help Guide walks families through every step that HICAP counselors can't handle: which form to file, which documents to gather, how to track the approval, what to do when the Part B premium keeps getting deducted after approval, and how to handle renewal paperwork. It doesn't replace HICAP — it picks up where HICAP leaves off.
