TennCare Renewal Packet: How Medicare Savings Program Redetermination Works

A renewal packet from TennCare can put a family into immediate panic mode. The envelope arrives with a strict 30-day deadline, and if your parent misses it, they could lose the Medicare Savings Program benefits that are paying $202.90 a month for their Part B premium. The good news: the process isn't as fragile as it first looks, and there's a safety net even if the deadline passes.

How Renewal Works

TennCare redetermines Medicare Savings Program eligibility every 12 months. The process has two stages:

Automatic (ex-parte) renewal: TennCare first tries to verify eligibility through electronic data matching — pulling income and asset data from the IRS, Social Security, and other sources. If everything checks out, your parent is automatically renewed and receives a confirmation notice. No action required.

Manual renewal via packet: If electronic verification is inconclusive — income changed, assets need fresh documentation, or a data match produces a discrepancy — TennCare mails a pre-populated renewal packet. The packet includes much of what TennCare already knows and asks your parent to confirm the details, correct anything outdated, and provide supporting documents.

The 30-Day Deadline

Your parent or their authorized representative has 30 days to complete, sign, and return the packet with any requested documentation. Follow the deadline stated in the packet. Submission options:

  • Online through TennCare Connect (tenncareconnect.tn.gov)
  • Mail to TennCare Connect, P.O. Box 305240, Nashville, TN 37230-5240
  • Fax to 1-855-315-0669

If the deadline passes without a response, TennCare issues a procedural closure notice and terminates MSP coverage. Social Security may begin deducting the Part B premium again after coverage ends.

The 90-Day Grace Period

Missing the 30-day deadline is not necessarily the end. Federal regulations provide a 90-day reconsideration grace period after procedural closure. If your parent returns the completed renewal packet and supporting documents within 90 days of termination, TennCare must re-evaluate eligibility without requiring a brand-new application.

This matters for situations like hospital stays, mail delivery problems, or an out-of-state adult child who didn't learn about the packet in time. TennCare must re-evaluate eligibility without requiring a new application. If your parent is approved, ask TennCare to confirm the effective date and contact SSA about any Part B premiums deducted during the gap.

If the 90 days have passed, your parent will need to submit a completely new application through TennCare Connect. There is no further grace period.

The COLA Protection

One of the most common renewal anxieties: your parent's Social Security check went up with the January Cost-of-Living Adjustment, and now they're worried they'll lose QMB or SLMB because their income is higher.

TennCare applies a COLA disregard from January 1 (when the Social Security increase takes effect) until TennCare adopts the updated Federal Poverty Level thresholds on April 1. During that window, the dollar increase from the COLA is completely excluded from the income calculation. This prevents anyone from being terminated due to a routine annual adjustment before the new poverty guidelines catch up.

Reporting Changes Between Renewals

Your parent is legally required to report income, household composition, or asset changes to TennCare within 10 days of the change. A new pension, a spouse's death, or selling a property that was previously excluded all count. Failure to report can lead to overpayment recovery later.

Report changes through TennCare Connect, by phone at 1-855-259-0701, or by mailing documentation to the same P.O. Box used for renewals.

Protecting Against Missed Renewals

The Tennessee Medicare Savings Programs & Extra Help Guide includes a renewal calendar and a checklist of documents to keep ready so the packet doesn't catch you off guard. If your parent has already been terminated, the guide walks through the 90-day reinstatement process and the full reapplication if that window has closed.
