Pennsylvania Medicare Savings Program Renewal and Redetermination

How Renewals Work in Pennsylvania

Pennsylvania's County Assistance Offices conduct periodic redeterminations to verify that Healthy Horizons (Medicare Savings Program) beneficiaries still meet income and resource limits. If electronic checks do not confirm continued eligibility or additional verification is needed, the CAO will send a redetermination packet with a return deadline.

The packet asks you to confirm or update:

  • Current monthly income from all sources (Social Security, pensions, VA benefits, any earned income)
  • Current bank balances, investment accounts, and other countable resources
  • Changes in household composition (spouse moved out, new dependent)
  • Changes in living arrangements or Pennsylvania residence

Respond by the deadline. If the case closes because you missed the renewal deadline, 42 CFR § 435.916 provides a 90-day reconsideration period after termination to submit the completed packet without filing a brand-new application.

The January COLA Problem at Renewal

The most common renewal complication happens in January. Social Security issues a cost-of-living adjustment that bumps your gross monthly income, but federal poverty guidelines — which set the MSP income limits — don't update until late February or March.

Pennsylvania handles this with an interim COLA disregard: during January and February, the CAO ignores the new COLA increase and keeps your countable income at last year's level. If your redetermination falls in this window, your January check amount shouldn't trigger a denial.

But errors happen. If you receive a notice saying your income exceeds the limit in January or February, check whether the CAO applied the COLA disregard. If they didn't, that's grounds for an appeal.

What to Do Before the Packet Arrives

Keep your documents current year-round so renewal isn't a scramble:

  • Save the most recent month's bank statements from every account — even ones with small balances
  • Keep your Social Security benefit verification letter (request one at ssa.gov or 1-800-772-1213)
  • Document any new medical expenses that could reduce countable income
  • Note any asset changes — if you sold a car or closed a CD, have the paper trail

If you moved within Pennsylvania or changed your phone number, update the CAO before the redetermination packet arrives. The packet goes to your address on file.

What Happens If You're Denied at Renewal

If the CAO determines you no longer qualify, the denial notice (Form PA/FS 162) gives you a standard 30-day appeal window from its mail date under 55 Pa. Code § 275.3(b)(1). The CAO or Bureau of Hearings and Appeals must receive the written appeal by the deadline. For a termination or reduction only, filing within 10 calendar days of the mail date maintains existing benefits while the appeal is pending.

Common denial reasons at renewal: income increased past the limit (new pension, higher Social Security), resources grew past $9,950/$14,910 (inheritance, account growth), or documentation wasn't returned in time.

If coverage closed because a renewal packet was late, use the 90-day reconsideration period to submit the completed packet without a new application. For another documentation denial, follow the appeal instructions on the notice and contact the CAO about submitting the missing records. If your income genuinely increased past QMB limits, check whether you still qualify for SLMB or QI, which have higher ceilings.

Shifting Tiers

Your renewal might move you between tiers rather than off the program entirely. A $100 increase in Social Security could push you from QMB ($1,350 limit) to SLMB ($1,351–$1,616) — you'd lose the deductible and copay coverage but keep the Part B premium assistance. That's still $202.90 per month you're not paying.

Similarly, someone near the QI ceiling ($1,816 single) who gets a COLA increase might fall off QI but qualify for Extra Help directly, which covers Part D drug costs even without MSP enrollment.

The Pennsylvania Medicare Savings Programs, Extra Help & PACE/PACENET Guide includes a renewal calendar and document checklist so you can prepare for redetermination well before the packet arrives.
