Healthy Horizons Pennsylvania — What It Is and How It Differs from Medicaid

Healthy Horizons Is Pennsylvania's Medicare Savings Program

If you've received a letter from PA DHS mentioning "Healthy Horizons," you're looking at Pennsylvania's brand name for its Medicare Savings Programs — the QMB, SLMB, and QI tiers that help low-income Medicare beneficiaries pay for premiums and cost-sharing.

The name causes real confusion. Families regularly mistake Healthy Horizons for full Medicaid, and that misunderstanding leads to wrong assumptions about asset limits, estate recovery, and what the card actually covers.

What Healthy Horizons Covers (and Doesn't)

Healthy Horizons runs on three tiers, each with different coverage:

QMB (Qualified Medicare Beneficiary) pays your Medicare Part A and Part B premiums, deductibles, copays, and coinsurance. It's the most comprehensive tier. Some QMB recipients also receive full Medical Assistance (category code PH), while others get cost-sharing coverage only (category code PG).

SLMB (Specified Low-Income Medicare Beneficiary) pays only your Part B premium — $202.90 per month in 2026. No deductible or copay coverage.

QI (Qualifying Individual) also pays only the Part B premium, but it's funded through a capped federal grant rather than as an open entitlement.

None of these tiers provide comprehensive medical coverage on their own. Healthy Horizons works alongside Medicare — it pays the costs that Medicare charges you, while Medicare remains your actual health insurance.

How Healthy Horizons Differs from Full Medicaid

Full Medical Assistance (Pennsylvania's Medicaid) covers doctor visits, hospital stays, prescriptions, and long-term care. For 2026, long-term-care Medical Assistance uses a special income limit of $2,982 per month. Its resource limit is $8,000 when income is at or below $2,982, and $2,400 when income is above $2,982.

Healthy Horizons has more generous limits. QMB's single-person income ceiling is $1,350 per month with a $9,950 resource limit. And critically, MSP-only benefits are exempt from Medicaid estate recovery under federal law. Enrolling in QMB, SLMB, or QI alone will not result in recovery of those MSP benefits. If someone with QMB and full Medical Assistance receives long-term-care services, recovery can apply to the long-term-care portion.

This distinction matters. Families regularly avoid applying for MSP because they assume the state will come after the house. For MSP-only assistance, that fear is unfounded; long-term-care Medical Assistance follows separate estate-recovery rules.

The ACCESS Card Question

When approved for Healthy Horizons, Pennsylvania issues an ACCESS card with your Medicaid Identification Number. This creates another layer of confusion — the card looks like a Medicaid card, but it's a verification tool, not a guarantee of full coverage.

Providers must check your active Program Status Code in the state's Eligibility Verification System before delivering services. The ACCESS card alone doesn't tell them whether you have QMB cost-sharing protection or just SLMB premium assistance.

Who Automatically Gets Healthy Horizons

Pennsylvania has a Section 1634 agreement with Social Security. If you receive SSI (Supplemental Security Income), you're automatically enrolled in full Medical Assistance and QMB without filing a separate application with DHS.

Everyone else needs to apply through one of the standard channels — COMPASS, a County Assistance Office visit, phone, or the Social Security MIPPA data transfer. All three MSP tiers also auto-qualify you for federal Part D Extra Help, which subsidizes prescription drug costs.
