South Carolina Medicare Savings Program Renewal: What to Expect at Redetermination
Getting approved for a Medicare Savings Program in South Carolina isn't a one-time event. SCDHHS redetermines eligibility every 12 months, and missing the renewal can cut off coverage — meaning the Part B premium deduction comes back and, for QMB enrollees, billing protections disappear.
How the Renewal Works
SCDHHS tries to renew your parent's MSP automatically first. This "ex parte" review checks state databases — Department of Employment and Workforce records, SSA data feeds — to verify that income and assets still fall within the limits. If the data confirms continued eligibility, the renewal is approved without your parent doing anything.
If SCDHHS can't verify eligibility automatically, they mail a Pre-Populated Annual Review Form. This arrives at your parent's address (or the authorized representative's address, if one is on file) with the information SCDHHS has on record. Your parent needs to review it, correct anything that's changed, and return it with updated bank statements and any other documentation of assets.
The 30-Day Deadline
SCDHHS gives your parent 30 calendar days to complete, sign, and return the renewal form. Follow the response deadline on the notice. Missing it can trigger a termination notice — coverage ends, the Part B premium deduction resumes, and Extra Help can be affected at the end of the calendar year.
This deadline is the most dangerous point in the annual cycle. During the 2023–2024 post-PHE Medicaid unwinding, South Carolina disenrolled thousands of beneficiaries who didn't return their review packets on time — many of whom were still eligible and simply missed the letter.
The 90-Day Grace Period
If your parent's coverage is terminated for not returning the renewal packet, there's a 90-day window to recover. If the completed review form arrives at SCDHHS within 90 days of the termination date, the agency will re-evaluate eligibility without requiring a brand-new application. After 90 days, your parent starts over with a fresh Form 3400.

Practical Steps to Protect Coverage
Watch the mail. The renewal packet doesn't arrive on the same date every year, and SCDHHS doesn't send reminders. If your parent moved or mail is delayed, contact SCDHHS promptly to confirm the response deadline.
Keep bank statements organized. The renewal packet will ask for current asset documentation. Having recent bank statements, CD records, and life insurance details ready avoids scrambling.
Use an authorized representative. If you're managing your parent's benefits remotely, make sure Form 1282 is on file so SCDHHS sends the renewal packet to your address too. Missing a renewal because a letter sat unopened at a parent's house is preventable.
