South Dakota Medicare Savings Program Renewal: How to Keep Your Benefits

Getting approved for QMB, SLMB, or QI is the hard part. Losing those benefits because a renewal form got buried in junk mail is the painful part. South Dakota DSS requires annual recertification, and the process catches families off guard more often than you'd expect.

How the Renewal Works

DSS first tries an ex parte renewal — verifying eligibility by cross-referencing state databases like wage records and quarterly SSA data. If the system confirms that income and resources still fall within limits, the renewal goes through automatically with no paperwork required.

When DSS can't verify eligibility through electronic records alone, it mails a pre-populated renewal form to the beneficiary's address on file. The form arrives pre-filled with the information DSS has, and the beneficiary needs to:

  1. Review the pre-filled data for accuracy
  2. Correct anything that's changed (income, address, bank accounts)
  3. Sign the form
  4. Attach updated financial documentation — typically recent bank statements
  5. Return everything within the 30-day response window

Miss that 30-day window, and DSS closes the case. Benefits stop.

The Junk Mail Problem

The renewal packet arrives in a state government envelope that looks like every other DSS mailing. Seniors with cognitive decline, vision problems, or simply a cluttered mailbox can miss it entirely. For families where an adult child lives out of state, the form might sit on a kitchen counter for weeks.

Two preventive measures help:

Designate an authorized representative. Complete Section 4 and the disclosure and signature sections of Form 270. DSS uses that designation to communicate with the representative about the case. Confirm with DSS whether renewal notices will be sent to the representative, and keep the beneficiary's mailing address current.

Report address changes within 10 days. If the beneficiary moves, changes a mailing address, or starts receiving mail at a different location, DSS must be notified within 10 calendar days. A renewal form sent to an old address doesn't reach anyone.

What to Report Between Renewals

Beneficiaries have a legal obligation to report specific changes to DSS within 10 calendar days:

  • Changes in income (a Social Security COLA increase, a new pension, a part-time job)
  • Changes in resources that push total assets above $9,950 (individual) or $14,910 (couple)
  • Changes in address or living arrangements
  • Changes in marital status or household composition

If you missed a reporting deadline, contact DSS promptly to report the change and ask how it affects the case.

The COLA Disregard: January Protection

Every January, Social Security benefits increase with the Cost-of-Living Adjustment. That raise can push a beneficiary's gross income above the prior year's MSP threshold — but the new federal poverty guidelines don't take effect in state regulations until around April.

South Dakota applies the mandatory COLA disregard during this gap. The dollar amount of the January COLA increase is excluded from countable income from January 1 through the date the updated poverty guidelines take effect. This prevents a temporary benefit loss every winter.

If Benefits Are Closed: The 90-Day Grace Period

A procedural closure — benefits terminated because the renewal form wasn't returned — isn't the end. If the completed renewal packet and verifications are submitted within 90 days of the closure date, DSS can reopen the case without requiring a brand-new application.

After that 90-day period, contact DSS about whether to submit a new application. Any new application is subject to the standard 45-day processing period.

The South Dakota Medicare Savings Programs & Extra Help Guide includes a renewal calendar and tracking system so families can anticipate the annual review, prepare documents in advance, and respond before the deadline passes.
