QMB Billing Protections in Missouri

The Federal Rule

Under 42 CFR 447.15, healthcare providers who accept Medicare are prohibited from billing QMB enrollees for Medicare deductibles, coinsurance, or copayments. This includes hospitals, doctors, labs, imaging centers, and any other Medicare-participating provider.

The prohibition applies to all Medicare cost-sharing amounts that QMB covers. If Medicare approves a service and there's a $200 coinsurance charge, the provider cannot send you a bill for that $200. Under Missouri's lesser-of reimbursement method, MO HealthNet may pay some or none of the remaining cost-sharing; the provider must write off any unpaid amount. If you receive a Part A hospital deductible bill for $1,676 while enrolled in QMB, that bill is illegal.

Why Missouri QMB Enrollees Still Get Bills

The billing protection is clear in federal law, but compliance on the provider side is inconsistent. Common reasons bills still arrive:

The provider's billing system didn't update. FSD's approval takes time to flow through CMS's systems to the provider's billing software. A provider who checks your coverage on the date of service might not see QMB status yet, and their automated billing cycle sends the statement.

The provider's billing staff doesn't know the rule. Smaller practices and hospital billing departments sometimes treat QMB like a secondary insurance that might not cover everything, rather than a federal prohibition on collecting cost-sharing from the patient.

Collections on old balances. Bills from before QMB coverage began are not protected — QMB only covers cost-sharing on services rendered while you were actively enrolled. If a balance predates your QMB effective date, the provider can collect on it.

What to Do When a Bill Arrives

Don't ignore it, and don't pay it. Take these steps:

Verify your QMB status was active on the date of service. Log into myDSS or call FSD at 855-373-4636 to confirm your coverage was active when the service was provided.

Contact the provider's billing department. Tell them you're enrolled in QMB and that billing you for Medicare cost-sharing violates 42 CFR 447.15. Many billing departments resolve this once they verify your QMB status in CMS's systems.

Provide documentation. If the billing department asks for proof, supply your FSD approval letter or an eMOMED printout showing active QMB coverage on the date of service.

File a complaint if the provider doesn't stop. Contact 1-800-MEDICARE (1-800-633-4227) to report a provider who continues to bill a QMB enrollee for protected cost-sharing. CMS can take compliance action against the provider.

Keep records. Save copies of every bill, your correspondence with the billing department, and your QMB coverage verification. If a provider sends a balance to collections, these records are your defense.
