The PHN's Role in Elderly Care

The public health nurse (PHN) is the front door to community health services for older people in Ireland. They work out of local health centres and operate within the HSE's Community Healthcare Networks, covering defined geographic areas.

For families arranging care for an aging parent, the PHN is typically the first professional you'll deal with — and often the person who conducts the interRAI Care Needs Assessment that determines home support hours.

How to Request a PHN Visit

To arrange a visit directly:

• Call your parent's local health centre and ask how to contact the PHN assigned to their area
• Your parent's GP can make the referral
• You can submit a Home Support Application Form, which triggers a PHN assessment as part of the process

If your parent is being discharged from hospital, ask the discharge coordinator how the PHN team will be involved in the transition plan.

What Happens During the Visit

The PHN assesses your parent's overall health status and living situation. For elderly assessments, this includes:

Physical health review: mobility, balance, recent falls, weight changes, skin condition, continence. The PHN checks whether your parent can manage stairs, transfer from bed to chair, and handle personal hygiene independently.

Cognitive screening: memory, confusion, orientation, safety awareness. If the PHN suspects cognitive decline, they may recommend follow-up through your GP.

Medication review: what prescriptions your parent takes, how they're stored, whether a blister-pack system is in place, and whether doses are being missed.

Home environment: fall hazards, bathroom accessibility, heating, kitchen safety, fire detection. The PHN notes anything that could warrant a referral to occupational therapy or the local authority's housing adaptation grants.

Social and support assessment: who currently helps your parent, how often, and whether informal carers (family members) are coping.

The PHN and Home Support Applications

When a home support application is submitted, the PHN is often the person who conducts the interRAI assessment — the formal clinical evaluation that decides how many HSE home support hours your parent receives.

This means the PHN visit and the care needs assessment are frequently the same appointment. If the PHN visits for a general welfare check and identifies significant care needs, they can help initiate the home support application; the application can also be completed by the PHN.

The PHN also handles the ongoing review of care plans. If your parent's condition changes — a fall, cognitive decline, a new diagnosis — the PHN can request a reassessment of the home support allocation without a fresh application.

What the PHN Cannot Do

The PHN is a clinical professional, not a home help worker. They conduct assessments, coordinate referrals, manage clinical nursing care (wound dressing, catheter care, injections), and oversee care plans. They do not provide the daily personal care visits — that's what the home support workers are for.

The PHN also cannot override HSE hour allocations. If you disagree with the number of hours your parent was approved for, the process is a formal written appeal to the local HSE Home Support Office within 20 days of the decision.

Making the Most of the Visit

Be present when the PHN visits, if possible. Your parent is likely to understate their difficulties — this is normal and nearly universal. You can provide context the PHN wouldn't otherwise see: the falls that happen at night, the meals being skipped, the confusion episodes your parent won't mention.

Have a written list of your parent's medications, recent hospital visits, and specific incidents of concern. Concrete, dated examples carry more weight than general impressions.
