Hospital Discharge Home Care Ireland: How to Arrange Support Before Your Parent Leaves Hospital

Why Hospital Discharge Is a Crisis Point

When an elderly parent is admitted to hospital — a fall, a stroke, a severe infection — the family gets pulled into care coordination with almost no notice. The hospital wants the bed back. The discharge team is pressing for a plan. And your parent may not be safe to go home without support that didn't exist before the admission.

This is the single most common trigger for families entering the HSE Home Support system. The good news: hospital discharge applications move faster than community ones.

The Hospital Discharge Pathway

For patients being sent home, the HSE Home Support application works differently than the standard community route:

The discharge coordinator initiates the referral. The hospital's medical social worker or discharge planner submits the home support referral during the hospital stay — not after discharge. This is a critical difference from the community pathway, where the family submits the 24-page form and waits 2–6 weeks.

The assessment can happen pre-discharge. An interRAI assessment or clinical evaluation may be conducted while your parent is still in hospital, or scheduled for the first days after they return home. The hospital team provides clinical notes that accelerate the process.

Turnaround is measured in days, not weeks. Hospital discharge applications are expedited because delayed discharges cost the acute care system significant resources. This gives families leverage the community route doesn't have.

What to Ask the Discharge Team

Before your parent leaves hospital, make sure you've clarified:

  • Has a home support referral been submitted? If not, ask the medical social worker to initiate one immediately.
  • What level of care does the team recommend? The discharge summary should specify whether your parent needs personal care, nursing visits, equipment, or all three.
  • Is a care needs assessment scheduled? Ideally the interRAI assessment happens before discharge or in the first days after your parent returns home.
  • What equipment is needed? Ask whether the HSE Community Services scheme can provide equipment such as patient hoists, specialised chairs, or walking aids. The research supports access for Medical Card and Long-Term Illness Card holders following a formal Occupational Therapist or Physiotherapist assessment, so confirm eligibility before discharge.
  • Who is the named public health nurse? The PHN coordinates community services post-discharge. Get the name and direct phone number.

The Unsafe Discharge Risk

Hospitals face pressure to free acute beds, and discharge teams may push for a date before home support is actually in place. If your parent would be unsafe at home — unable to toilet independently, at high fall risk, no one available to help with medication — raise this with the team.

Put your concerns in writing. A letter or email to the consultant and medical social worker stating specific safety risks gives the team a clear record to consider in discharge planning.

That said, "unsafe" means clinically unsafe — not merely inconvenient. Raising a concern does not itself guarantee that discharge will be delayed; the team must consider the immediate clinical risks and the planned supports.

Bridging the Gap with Private Care

Even with an expedited hospital referral, HSE home support hours may not start immediately after discharge. Workforce shortages and scheduling delays mean there's often a gap between leaving hospital and receiving funded care visits.

Private home care agencies can sometimes step in quickly, subject to provider availability. At €30–€35 per hour, this is expensive short-term, but it prevents the worst outcome: your parent going home to an empty house with no support after a serious medical event.

Private care costs during this transition period may qualify for Section 467 tax relief if your parent meets the "totally incapacitated" test, with deductions up to €75,000 per year at the marginal tax rate.

After Discharge: The Early Home Period

Use the early period after hospital discharge to confirm that the planned supports are in place. During this period:

  • Confirm the PHN has visited or has a visit scheduled
  • Verify medication is dispensed in blister packs and the home support worker knows the prompting schedule
  • Check that prescribed equipment (grab rails, shower seat, bed rail) has been installed
  • Keep the hospital discharge summary accessible — every professional visiting your parent needs to see it
