Oklahoma DHS Form 08MP003E Application
What Form 08MP003E Is and Why It Matters
Form 08MP003E is the Application for Health Coverage and Help Paying Costs for Aged, Blind, or Disabled individuals in Oklahoma. It's the form OKDHS uses to determine eligibility for the Medicare Savings Programs (QMB, SLMB, QI-1, QDWI) and for full SoonerCare Medicaid under the ABD category.
This is not the same as the general SoonerCare application. The general health coverage application through mySoonerCare handles MAGI-based Medicaid for children, pregnant women, and parents. Aged and disabled applicants — the population seeking help with Medicare costs — need Form 08MP003E, filed through OKDHSLive or a county OKDHS office.
Filing the right form matters because it triggers evaluation of the MSP tiers. If you submit the wrong application, your parent may be assessed only for full SoonerCare, denied for being over-income, and never told about QMB or SLMB.
Where to Get the Form
Online: The electronic equivalent is built into OKDHSLive (okdhslive.org). When you start a new application and indicate the applicant is aged 65+ or disabled, the system should route you to the ABD pathway.
Paper: Download Form 08MP003E from the OKDHS website or pick up a copy at any county Human Services Center. Oklahoma has offices in all 77 counties.
By phone: Call 1-800-987-7767 or 405-522-5050 to request a form by mail or start an application over the phone.
Key Sections to Get Right
The Medicare Checkbox
The form includes a screening section asking whether you need help with Medicare costs. Check this box. If it's left unchecked, the caseworker may evaluate only full SoonerCare eligibility and skip the MSP assessment entirely. This single checkbox is responsible for many of the "I was told I don't qualify" stories that circulate online.
Income Reporting
Report all income sources at their gross monthly amounts:
- Social Security retirement or disability benefits (the gross figure from the benefit letter, before Part B deduction)
- Pensions and annuities
- Earned income (wages or net self-employment after IRS-allowable expenses)
- VA payments (service-connected disability is counted; Aid & Attendance is excluded)
Do not subtract Medicare premiums, tax withholdings, or voluntary garnishments from the income figures. OKDHS starts from gross and applies its own disregards.
Asset Reporting
List the current balance of every countable financial account: checking, savings, CDs, money market accounts, IRAs, and 401(k)s. Include the account numbers.
Answer each asset question the form asks; don't omit an item solely because you believe it's exempt. The primary home, one primary vehicle used for household transportation, personal belongings, burial plots, and qualifying irrevocable funeral contracts are excluded from the MSP resource limit.
For joint accounts shared with an adult child, report the full balance. The caseworker counts 100% as the applicant's unless the child can document that the funds are solely theirs.
Signature Requirements
The applicant must sign the form (or their authorized representative with legal documentation — POA, guardianship, or OKDHS Form 08MP001E). An unsigned form is not a valid application and doesn't establish an application date.

What Happens After Submission
The date OKDHS receives a signed Form 08MP003E (or electronic submission through OKDHSLive) establishes the official application date. This date is legally significant:
- SLMB and QI-1 can provide up to 3 months of retroactive premium coverage before the application month
- OKDHS has 45 calendar days from this date to process the application
After submission, OKDHS runs automated data queries through BENDEX (Social Security income), SDX (SSI status), and AVS (bank account verification). If these electronic checks don't cover everything, the caseworker sends Form 08MA005E requesting additional documentation. You typically have 10 to 14 days to respond.
Common Mistakes That Delay Processing
Not providing bank statements upfront. Even though OKDHS queries the AVS system, paper bank statements for the last 30–60 days are almost always requested. Submitting them with the initial application avoids the back-and-forth.
Confusing the two portals. Applying through mySoonerCare instead of OKDHSLive routes the case through the wrong eligibility track.
Forgetting representative authorization. If the adult child is filing on behalf of a parent, the case stalls until Form 08MP001E (or a POA/guardianship order) is on file.
The Oklahoma Medicare Savings Programs & Extra Help Guide walks through Form 08MP003E section by section, with a pre-submission checklist so nothing is missing when you file — because a complete application on day one is the fastest path to approval.
