Nebraska Medicare Savings Program Renewal: How to Keep Your Benefits
Losing MSP coverage at renewal is almost always a paperwork problem, not an eligibility problem. Nebraska DHHS redetermines eligibility every 12 months, and when the automated renewal can't verify your income and assets electronically, you get a packet in the mail. Miss the 30-day deadline on that packet and DHHS can close the case; your Part B premium may resume once the MSP buy-in ends.
How Automated Renewals Work
DHHS attempts an ex-parte renewal first. The agency queries federal and state electronic data sources — Social Security Administration records, state wage databases, and financial data matching systems — to verify that your income and resources still fall within MSP limits.
If the electronic data confirms eligibility, your coverage renews automatically. You receive no paperwork and no action is required. Many beneficiaries don't even realize a renewal happened.
When You Get the Renewal Packet
Automated verification fails when electronic data is incomplete, inconsistent, or unavailable — common with pension income, investment accounts, or recent changes in bank balances. DHHS mails a pre-populated renewal form with the information they already have. Your job is to correct anything that's wrong and attach updated proof.
Return the form within 30 days with:
- Current bank statements (all accounts, full month)
- Any pension or annuity statements that changed since the last renewal
- Updated income documentation if Social Security benefits changed (the annual COLA adjustment is the typical trigger)
Submit through iServe Nebraska by uploading the documents, or mail everything to the DHHS Document Imaging Center at P.O. Box 2992, Omaha, NE 68103-2992.
The Social Security COLA Timing Trap
Every January, Social Security benefits increase with the Cost-of-Living Adjustment, but the new Federal Poverty Level guidelines that MSP limits are based on don't publish until February or March. To prevent seniors from losing coverage during this gap, federal law requires Nebraska DHHS to disregard the COLA increase for MSP eligibility until the month after the new FPL is published.
If you receive a renewal notice in January or February showing your income above the limit because of the COLA, the COLA disregard should protect you. Contact DHHS at (855) 632-7633 if the renewal form doesn't reflect this.

What Happens If Coverage Lapses
If you miss the 30-day renewal window and DHHS closes your MSP case:
- The Part B premium deduction may resume on your Social Security check once MSP coverage ends.
- Extra Help may also lapse, causing prescription copays to jump.
- You have a 90-day grace period to submit late renewal documentation and get reinstated without filing a completely new application.
During those 90 days, gather the same documents the renewal required and submit them. Contact DHHS to confirm the MSP effective date and whether the state will restore the Part B buy-in for the lapse; if premiums were deducted, ask SSA whether a refund is due.
After 90 days, the grace period expires and you must file a new application from scratch — new MLTC-64 or a fresh iServe submission.
Preventing Renewal Problems
Keep your contact information current in iServe. DHHS mails the renewal packet to the address on file — if you've moved without updating, the packet goes to the wrong address and the deadline passes.
Set a calendar reminder for the month before your annual renewal date. Check iServe for any action alerts. Having bank statements and income documentation ready before the packet arrives cuts turnaround time dramatically.
