Mississippi Medicare Savings Program Documents Needed
The Documents DOM Actually Needs
Missing documents are the single most common reason MSP applications stall in Mississippi. DOM generally has 45 days to decide most applications or 90 days when a disability determination is required. Federal rules allow delays when applicant or examiner delay prevents a decision. If the requested documents aren't provided by the deadline, the application can be denied procedurally — even when your parent clearly qualifies.
Here's what to gather before you submit Form DOM-300:
Identity and age verification — one of:
- Mississippi driver's license or state ID
- Birth certificate
- Valid U.S. passport
Medicare entitlement — required:
- Copy of the red-white-and-blue Medicare card, front and back
- Medicare Beneficiary Identifier (MBI) number
- Part A and Part B start dates (printed on the card)
Income verification — the most critical section:
- Social Security Benefit Adjustment Letter showing gross monthly amount (not the net deposit)
- Monthly pension statements
- VA benefit award letters
- Annuity pay stubs
- For earned income: recent pay stubs or self-employment records
Citizenship or immigration status — one of:
- U.S. passport
- Birth certificate
- Green card (Form I-551) or other qualifying immigration documentation
Mississippi residency — one of:
- Utility bill in the applicant's name
- Lease or rental agreement
- Property tax receipt
- Mortgage statement
The Gross vs. Net Income Mistake
DOM counts gross Social Security income — the full benefit amount before Part B premiums, tax withholdings, or garnishments are deducted. If your parent's bank deposit is $1,117.10 after only the standard 2026 Part B premium of $202.90 is deducted, their gross Social Security is $1,320. DOM evaluates the $1,320 figure.
The document that shows the correct number is the Social Security Benefit Adjustment Letter (sometimes called the COLA notice). This letter states the gross monthly benefit. If your parent can't find it, they can request a Benefit Verification Letter from SSA at 1-800-772-1213 or through my.ssa.gov.
Do not submit a bank statement showing the net deposit amount — DOM needs the gross figure to apply the $50 Mississippi income disregard correctly.
The Authorized Representative Section
For an ABD case, Form DOM-300 does not itself appoint an authorized representative. Use Form DOM-301 to authorize DOM to release case information. To appoint someone to act as an authorized representative, the applicant designates them in writing on Form DOM-302A. DOM also has Form DOM-302B for a self-designated representative and Form DOM-302C for a legal representative with authority to make healthcare decisions.
If your parent can sign and only wants you to receive case information, use DOM-301. If they want you to act for them, use the form that matches their situation and your authority. Without the appropriate release or representative form, DOM cannot discuss the case with you when you call to check status.

Why the Form Asks About Assets
Form DOM-300 is a multi-program application covering full Medicaid, the E&D Waiver, institutional care, and MSPs. The asset sections exist for programs that enforce asset limits. For QMB, SLMB, and QI applicants, Mississippi's eligibility workers bypass the resource evaluation entirely — the state eliminated asset testing for these programs.
Fill in the sections as requested, but don't let them discourage you from applying. Reporting $50,000 in savings won't cause a denial for a standalone MSP application.
