How to Apply for Alabama Medicare Savings Program

The application for Alabama's Medicare Savings Programs is a single form — Form 210 — and it can be submitted by mail, fax, phone, or in person at a District Office. The process itself is straightforward, but small mistakes cause most of the delays: sending the form to the wrong address, attaching the wrong income document, or using net Social Security instead of gross.

Get the Right Form

Form 210 is the streamlined Medicare Savings Program application. It asks only for identity, household income, and Medicare details — nothing about assets, because Alabama does not count resources for MSP eligibility.

You may also use Form 207 (the general Aged, Blind, and Disabled Medicaid application), but Form 210 processes faster because it skips the sections that do not apply to MSP cases. If you see asset-declaration fields on a form, they are non-binding for MSP applicants.

A common source of confusion: some older resources reference "Form 211." The correct MSP application is Form 210, available from the Alabama Medicaid Agency website or any District Office.

Four Ways to Submit

  1. Mail — Send the completed, signed Form 210 to: Alabama Medicaid Agency, Eligibility Division, P.O. Box 5624, Montgomery, AL 36103-5624. Use certified mail with return receipt so you have a dated filing record.

  2. District Office — Hand-deliver the form to any of Alabama's 11 Medicaid District Offices (Auburn, Birmingham, Decatur, Dothan, Florence, Gadsden, Huntsville, Mobile, Montgomery, Selma, or Tuscaloosa). The office that covers the applicant's county of residence handles the case.

  3. Phone — Call the Alabama Medicaid Recipient Call Center at 1-800-362-1504 (Monday through Friday, 8:00 a.m. to 4:30 p.m.) to initiate an application by phone.

  4. Fax — Fax the signed form to the assigned District Office's intake fax line. Keep the fax transmission confirmation as proof of filing date.

The state portal at my.alabamamedicaid.gov is primarily structured for MAGI and family Medicaid cases. Standalone Form 210 submissions cannot easily be tracked through the portal.

Required Documents

Attach these with the application:

  • Medicare card (the red, white, and blue card) or a Social Security award letter showing Medicare entitlement
  • Income verification — the SSA-1099 or current benefit adjustment letter showing gross monthly Social Security (not the net deposit amount). For non-Social Security income, provide pension statements, retirement notices, or consecutive pay stubs for the last 30 to 60 days
  • Government-issued photo ID and proof of Alabama residency (utility bill or lease)

Do not attach bank statements, property records, or retirement account balances. Alabama does not evaluate assets for MSP applications.

Processing Timeline

Alabama Medicaid must process MSP applications within 45 calendar days from receipt. If the agency needs additional documentation, it sends a Request for Information notice with a 10-to-14-day compliance deadline. Missed deadlines can result in the application being closed, so respond promptly and keep copies.

After approval, it takes an additional 60 to 90 days for SSA to update the beneficiary's payment record and stop deducting the Part B premium. SSA issues a retroactive lump-sum refund of all premiums deducted during the active MSP period — no separate refund request is needed.

Filing on Behalf of a Parent

If an adult child is filing Form 210 for a parent, Alabama Medicaid requires a completed Appointment of Representative form signed by the applicant. If the parent is physically or mentally unable to sign, the representative must provide a copy of a durable power of attorney, a court guardianship order, or a medical certification of incapacity. Without these, an immediate family member acting as a sponsor may sign under Alabama Administrative Code r. 560-X-25-.04(2).
