Alabama Medicaid Form 210: Medicare Savings Program Application Walkthrough

If you've been told to "fill out Form 211" for Alabama's Medicare Savings Programs, you're actually looking for Form 210 — the dedicated MSP application. The number mix-up is everywhere online, in waiting rooms, even from some caseworkers. Form 210 is shorter and simpler than the general Medicaid application (Form 207), and it's the fastest path to getting QMB, SLMB, or QI-1 benefits started.

What Form 210 Covers

Form 210 is Alabama Medicaid's streamlined application specifically for Medicare Savings Programs. Unlike the broader Form 207 (the general Aged, Blind, and Disabled application), Form 210 asks only for information relevant to MSP eligibility: your identity, household income, and Medicare details.

You can use Form 210 to apply for any of the three MSP tiers:

  • QMB — pays Part B premium, Part A premium (if applicable), deductibles, and coinsurance
  • SLMB — pays Part B premium only
  • QI-1 — pays Part B premium only (federal block grant funded)

Alabama Medicaid determines which tier you qualify for based on your income. You don't need to pick one — the agency assigns the highest tier your income supports.

Sections You'll Complete

Form 210 focuses on three categories of information:

Personal identification. Full legal name, Social Security number, date of birth, current Alabama address, and phone number. If you're applying as a couple, both spouses' information goes on the same form.

Medicare details. Your Medicare Beneficiary Identifier (MBI) from your red, white, and blue Medicare card. The form asks whether you have Part A, Part B, or both — and what premiums you're currently paying.

Household income. Every source of gross monthly income: Social Security benefits (use the gross amount from your SSA-1099, not the direct deposit amount after deductions), any pension or annuity payments, wages, and VA benefits. Alabama applies the standard $20 general income disregard and, for earned income, an additional $65 plus half the remainder.

Older-form asset fields. Older versions of Form 207 or general Aged, Blind, and Disabled applications may show questions about assets. Alabama excludes resources from QMB, SLMB, and QI-1 eligibility, so an application cannot be denied because of excess assets.

Documents to Attach

Gather these before you start:

  • Copy of your Medicare card (front and back)
  • Most recent Social Security benefit letter or SSA-1099 showing gross monthly amount
  • Pension or retirement income statements (if applicable)
  • Pay stubs for the last 30–60 days (if you have earned income)
  • Government-issued photo ID
  • Proof of Alabama residency (utility bill or lease)
  • Immigration documentation (if non-citizen)

Alabama Medicaid uses electronic data matching for income verification when possible, so paper documents are a backup — but having them ready prevents processing delays.

Four Ways to Submit

You can file Form 210 through any of these channels:

  1. Mail directly to Alabama Medicaid Agency, Eligibility Division, P.O. Box 5624, Montgomery, AL 36103-5624. Send by Certified Mail with Return Receipt to create a paper trail.
  2. In person at any of Alabama's 11 regional Medicaid District Offices.
  3. Fax to your regional District Office's intake fax line.
  4. Phone — call the Recipient Call Center at 1-800-362-1504 (Monday–Friday, 8:00 a.m.–4:30 p.m.) to initiate an application by telephone.

The my.alabamamedicaid.gov portal handles full Medicaid and some ABD applications, but its capability for standalone Form 210 MSP cases is limited. Paper, fax, or phone remain the reliable options.

Filing on Someone Else's Behalf

If you're completing Form 210 for a parent or relative, Alabama requires an Appointment of Representative form (Form 560-X-28-.01(16)) signed by the applicant. When the applicant can't sign due to incapacity, you'll need a Durable Power of Attorney, court guardianship order, or medical certification. An immediate family member acting as sponsor can execute the application under rule 560-X-25-.04(2) without a formal POA.

What Happens After You Submit

Alabama Medicaid must process MSP determinations within 45 calendar days from receipt of a signed application. You'll receive a written notice specifying which tier you're approved for (QMB, SLMB, or QI-1), the effective date, and your appeal rights if denied.

The Alabama Medicare Savings Programs & Extra Help Guide walks through every line of Form 210 with Alabama-specific income calculation worksheets, a pre-submission checklist, and the complete district office directory so you know exactly where to send your application.
