Delaware Medicare Savings Program Processing Time: How Long Does It Take?
You have submitted your Medicare Savings Program application through Delaware ASSIST or dropped it off at a DSS State Service Center. Now the question on everyone's mind: how long until you actually see relief on that Social Security check?
The Official Processing Window
Delaware DSS has 45 days from the date it receives a complete application to issue a written decision. A complete application includes the application and required verification documents — such as proof of income, Medicare entitlement, identity, and residency.
If DSS needs additional verification, it may send a follow-up letter asking for missing documents. Respond by the stated deadline to avoid delays.
For disability-based applications, the processing window can be up to 90 days.
What Happens After Approval
Once DSS approves your application, three things happen in sequence — and each has its own timeline:
1. DSS issues a Notice of Decision. It states your MSP category (QMB, SLMB, or QI), your effective coverage start date, and the countable income the caseworker calculated.
2. DMMA transmits your eligibility to CMS. The Division of Medicaid and Medical Assistance sends an automated file to the federal Centers for Medicare & Medicaid Services to establish your state "Buy-In" status. This is what tells the federal system that Delaware is now paying your Part B premium.
3. SSA updates your payment record. CMS passes the information to Social Security, which stops deducting the Part B premium from your monthly check. Because of batch processing schedules between DMMA, CMS, and SSA, this step typically takes 60 to 90 days after approval.
Once the Buy-In is fully established, SSA issues a retroactive lump-sum refund for all Part B premiums that were withheld from your Social Security check after your coverage start date. At $202.90 per month in 2026, even a two-month lag means a refund of over $400.
QMB vs SLMB and QI Start Dates
The effective date of your coverage depends on which program you qualify for:
- QMB: Coverage begins the first day of the month after the approval decision. QMB cannot be applied retroactively.
- SLMB and QI: Coverage can be backdated up to three months before the month you applied, as long as you met all eligibility criteria during those months. This means your retroactive premium refund could cover an extra quarter.

Common Causes of Delays
Missing verifications can delay an application. DSS may send a follow-up request, so return requested documents by the stated deadline.
Other common slowdowns:
- Spousal income requires deeming calculations. When one spouse is on Medicare and the other is not, DSS applies spousal deeming rules to determine countable income.
- MIPPA-triggered applications. If you applied for Extra Help through Social Security, SSA automatically transmits your data to Delaware as an MSP application. This triggers a follow-up letter from DSS requesting missing verifications, adding a round trip of mail time.
- QI-1 federal allotment limits. QI operates under a capped annual federal block grant. Applicants should apply early in the year and respond promptly to annual renewal notices.
How to Check Your Application Status
Log into your ASSIST account at assist.dhss.delaware.gov to see case updates. You can also call the DSS Customer Relations Unit at 1-866-843-7212 for a status check over the phone.
The processing standard is 45 days after DSS receives a complete application. If your application remains pending, call Customer Relations to check its status and ask about next steps.
