Delaware Medicare Advantage vs Medicare Savings Program: What's the Difference?

The question comes up constantly: should my parent stay on Original Medicare or switch to Medicare Advantage? And does either choice affect their eligibility for the Medicare Savings Programs? The answer is simpler than most people expect — because these two things are not competing options. They operate on different tracks entirely.

What Each One Actually Does

Medicare Advantage (Part C) is a way to receive your Medicare benefits. Instead of Original Medicare (Parts A and B administered directly by the federal government), a private insurance company delivers your covered services through its own network. Many MA plans bundle Part D prescription coverage, add dental or vision benefits, and cap annual out-of-pocket spending. You still pay the Part B premium, and some plans charge an additional monthly premium.

Medicare Savings Programs (QMB, SLMB, QI) are state-administered financial assistance programs that help pay for Medicare costs. Delaware runs them through the Division of Social Services and the Division of Medicaid and Medical Assistance. They cover your Part B premium, and in the case of QMB, your Part A premium, deductibles, and coinsurance as well.

One is your insurance delivery system. The other is financial help paying for it. They are not alternatives to each other.

Can You Have Both?

Yes. You can be enrolled in a Medicare Advantage plan and also receive Medicare Savings Program benefits simultaneously. Delaware does not require you to be on Original Medicare to qualify for QMB, SLMB, or QI. The eligibility criteria — income below the Federal Poverty Level thresholds and Medicare Part A entitlement — apply regardless of whether you get Medicare through Original Medicare or an MA plan.

If you have QMB and a Medicare Advantage plan, Delaware pays your Part B premium (and Part A premium if applicable). The MA plan's own cost-sharing rules apply for services, but the QMB balance billing protections remain in effect — providers cannot bill you for Part A or Part B deductibles and coinsurance. In practice, many MA plans already cap these costs lower than Original Medicare would, so the overlap tends to work in the beneficiary's favor.

Where the Confusion Comes From

Insurance brokers often discuss Medicare Advantage as if it were an alternative to applying for state benefits. It is not. A broker earns a commission when they enroll someone in an MA plan. They earn nothing when someone applies for QMB through Delaware DSS. The incentive structure means brokers rarely mention that the state will pay a low-income senior's Part B premium regardless of which type of Medicare coverage they choose.

The Delaware Medicare Assistance Bureau (DMAB) — the state's official SHIP counseling agency — provides free, unbiased plan comparisons. Unlike commercial brokers, DMAB counselors do not earn commissions and will walk through both the plan choice and the MSP eligibility question together. Reach DMAB at 1-800-336-9500.

Which Matters More for a Low-Income Senior

For someone living on a fixed Social Security check near or below the poverty line, the Medicare Savings Program question is far more consequential than the MA vs Original Medicare choice. Getting approved for QMB can recover over $2,400 a year in Part B premiums alone, plus eliminate deductibles and coinsurance that can add thousands more.

The MA plan choice matters for network access, extra benefits, and annual cost caps. But it does not put $200 a month back into the Social Security check. The MSP does.

If your parent has not yet applied for a Medicare Savings Program, start there. The plan comparison can happen alongside — or after — the MSP application is in process.
