Delaware Medicare Savings Program vs Medicaid: Key Differences

Families regularly confuse Medicare Savings Programs with full Medicaid — and the confusion isn't academic. One triggers estate recovery in Delaware, the other doesn't. One covers nursing home care, the other doesn't. Mixing them up can lead families to either avoid a program they qualify for or expect coverage they won't receive.

What Medicare Savings Programs Cover

Delaware's Medicare Savings Programs (QMB, SLMB, QI) address Medicare costs specifically:

  • QMB pays Part A and Part B premiums, deductibles, coinsurance, and copays
  • SLMB and QI pay the Part B monthly premium ($202.90/month in 2026)

MSP enrollment does not give your parent access to the full Delaware Medicaid benefit package. A person with QMB-Only status receives Medicare cost-sharing help but cannot access Medicaid-covered services like adult dental, long-term care, managed care under the Diamond State Health Plan (DSHP), or waiver services.

What Full Medicaid Covers in Delaware

Full Delaware Medicaid — including the Aged, Blind, and Disabled (ABD) category — covers comprehensive medical care: doctor visits, hospital stays, prescriptions, long-term care (nursing home and community-based), adult dental, home health, personal care, and managed care enrollment through DSHP or DSHP-Plus.

Full Medicaid has different eligibility rules. While MSPs use only an income test (Delaware eliminated the asset test for QMB, SLMB, and QI), full Medicaid for long-term care enforces a strict resource limit of $2,000 for an individual and $3,000 for a couple in countable assets. Home equity, retirement accounts, and bank balances all factor in.

The Estate Recovery Question

Here's what drives the most anxiety: Delaware's Medicaid estate recovery program can seek repayment for certain Medicaid long-term-care services received at age 55 or older.

MSP-Only enrollment does not trigger estate recovery in Delaware. QMB, SLMB, and QI are classified as Medicare cost-sharing assistance, not comprehensive Medicaid services. If your parent receives only MSP benefits (and not full Medicaid), the state cannot file a claim against their estate.

This distinction matters for families who own a house. The fear that "Medicaid will take the house" applies to long-term care Medicaid, not to Medicare Savings Programs. A parent who qualifies for QMB because their income is below 100% FPL but has a paid-off home can receive MSP benefits without creating an estate recovery exposure.

Dual Eligibility: Having Both

Some beneficiaries qualify for both MSP and full Medicaid simultaneously. Delaware calls this "QMB Plus" or "SLMB Plus" — the person meets the MSP income criteria and separately qualifies for full ABD Medicaid or Long-Term Services and Supports.

A Full Dual Eligible gets Medicare cost-sharing protection through their MSP tier plus the full Medicaid benefit package including long-term care, managed care, and additional services. Estate recovery can apply to the full Medicaid services received, though not to the MSP cost-sharing component.

Income Test vs Asset Test Summary

Program Income Test Asset Test Estate Recovery
QMB ≤ 100% FPL None (eliminated) No
SLMB 100–120% FPL None (eliminated) No
QI 120–135% FPL None (eliminated) No
Long-term-care Medicaid Varies by category $2,000 individual / $3,000 couple Yes (covered long-term-care services received at age 55 or older)

Which to Apply For

Apply for the Medicare Savings Program first if the goal is Part B premium relief and Medicare cost-sharing help. MSP income limits range from 100% to 135% FPL, with no asset test or estate recovery on MSP benefits. Long-term-care Medicaid has separate eligibility rules and a $2,000 individual / $3,000 couple countable-asset limit.

The Delaware ASSIST portal screens for both simultaneously — one application evaluates all categories the applicant might qualify for. But understanding the difference upfront prevents surprise when MSP approval doesn't include a Medicaid card for dental or nursing home services.

Our Delaware Medicare Savings Programs, Extra Help & DPAP Guide explains the distinction in detail, including how dual eligibility works and when a separate Medicaid application makes sense alongside MSP enrollment.
