How to Apply for Medicare Savings Program in Delaware

Delaware doesn't have a separate form just for Medicare Savings Programs. The application runs through the state's combined benefits system, and if you don't know where to look in the portal, you'll end up screened for SNAP and Cash Assistance while the Medicare cost-help option sits untouched.

Two Ways to Apply

Online through Delaware ASSIST at assist.dhss.delaware.gov. Create an account (or log in if you have one), start a new application, and during the benefit selection step, choose "Help with Medicare Costs" or "Medical Assistance." The system screens simultaneously for QMB, SLMB, and QI based on the income information you enter.

Paper application using the Application for Social Service Benefits (Form DSS-100 or the combined DMMA Medical Application). There is no standalone MSP form — this general application is the only paper route. Pick one up at a Delaware DSS State Service Center or Social Services office, call 1-866-843-7212 to request one by mail, or download it from the DHSS website. Submit by mail, secure fax, or in-person drop-off.

Both channels feed into the same eligibility system. The online route lets you upload documents directly; the paper route requires mailing or faxing supporting documents separately if you don't submit them in person.

Documents You Need

Gather these before starting the application — incomplete submissions are the top cause of processing delays:

  • Photo ID — driver's license, state ID card, or U.S. passport
  • Medicare card — the red, white, and blue card, or an SSA Award Letter showing Part A entitlement
  • Proof of Delaware residency — driver's license with current address, utility bill, lease, or a resident statement
  • Income verification — current Social Security award letter (the one showing your 2026 monthly amount), pension statements, or pay stubs for the last 30 days
  • Authorized representative form — if you're applying for a parent, submit the Delaware DHSS Authorized Representative Designation Form (Form DSS-A1). An existing Power of Attorney works too, but must be filed with DSS separately

You do not need to document assets. Delaware has eliminated the resource test for QMB, SLMB, and QI. The application may still ask about bank accounts and property because it screens for other programs that do check assets — but for MSP purposes, caseworkers disregard those answers.

What Happens After You Submit

DSS has 45 days from receiving a complete application to issue a written Notice of Decision. The notice tells you which tier was approved (or the reason for denial), the effective coverage start date, and your appeal rights.

If the application is incomplete, DSS sends a request for the missing documents. The 45-day clock pauses until you respond. Missing the verification deadline can result in denial for procedural reasons — not because you didn't qualify, but because the file wasn't complete.

After Approval: The Timeline

QMB coverage starts on the first day of the month after the approval decision. It cannot be applied retroactively.

SLMB and QI coverage can be applied retroactively up to 3 months before the application month, as long as eligibility existed during those months.

Once DMMA transmits the approval to CMS, it takes 60 to 90 days for SSA to update its payment system. During that lag:

  • The Part B premium may still be deducted from your parent's Social Security check
  • SSA eventually issues a retroactive refund for all premiums withheld after the effective date
  • The refund arrives as a lump sum via check or direct deposit

The MSP approval also automatically triggers Extra Help (Part D Low-Income Subsidy) — no separate application required. CMS data exchange handles it, though pharmacy systems may take a few weeks to reflect the change.

If Something Goes Wrong

A denial notice includes detailed fair hearing appeal rights. You have 90 days from the date the notice is mailed to request a hearing; if the postmark is unavailable, Delaware allows a five-day grace period from the notice date for mail processing. The request must be received by the hearing office within the applicable deadline. To request "aid continuing," submit the hearing request during the ten-day timely-notice period, before the effective date on the notice. Email requests to [email protected]; follow the notice's instructions for other methods.
