How to Apply for the Medicare Savings Program in Rhode Island
Four Ways to Apply
Rhode Island's Department of Human Services (DHS) accepts Medicare Premium Payment Program (MPPP) applications through four channels. Every route leads to the same eligibility determination — pick whichever fits your situation.
Online through HealthyRhode (healthyrhode.ri.gov): Create an account, log in, and look for the "Medicare Premium Payment" option. This is critical — if you start a general Medicaid or MAGI application instead, the system routes your case through the wrong evaluation path. Select Medicare Premium Payment specifically.
Paper form by mail: Download the MPP-1 form from the EOHHS website or call DHS to have one mailed to you. Complete it and send to: RI Department of Human Services, P.O. Box 8709, Cranston, RI 02920-8787.
In person at a DHS field office: Walk into any regional office during business hours, or use the 24/7 exterior drop boxes at most locations. Offices are in Providence (125 Holden Street), Pawtucket (249 Roosevelt Avenue), Woonsocket (219 Pond Street), Wakefield (4808 Tower Hill Road, Suite G1), and Middletown (31 John Clarke Road).
By phone: Call 1-855-697-4347 (1-855-MY-RIDHS) on Monday, Tuesday, Thursday, or Friday between 8:30 AM and 3:00 PM. No Wednesday hours.
Documents You'll Need
Gather these before starting:
- Medicare card showing Part A and Part B coverage
- Social Security benefit letter — the current award notice showing your gross monthly benefit (not the deposit amount)
- Bank statements for all accounts covering the past one to three months
- Proof of Rhode Island residency — utility bill, lease, or state ID
- Proof of citizenship or lawful presence
- Pension or retirement income statements if you receive any
- Life insurance policy declarations if the total face value of whole life policies exceeds $1,500 (the cash surrender value counts as an asset)
Missing a document won't automatically reject your application, but DHS will mail a supplemental request with a deadline — typically 30 days. Miss that deadline and the case closes.
The HealthyRhode Portal: Common Pitfalls
The online portal works, but it has known friction points. File uploads can fail silently on mobile browsers. If you're uploading bank statements or income letters, use a desktop browser and confirm each file appears in your submission before finalizing.
If the portal glitches, don't wait — print the MPP-1 form and mail or drop off a paper copy. Your application date is the date DHS receives a complete submission, and that date matters: QI coverage can be retroactive to the month of filing.

What Happens After You Submit
DHS has 45 calendar days to issue an eligibility determination. You can check your case status by logging into HealthyRhode or calling the DHS Call Center.
If you're approved for QMB, coverage starts the first day of the month after the approval decision. QI coverage can start the month you filed — and can even go back up to three months within the same calendar year, provided you met the income and asset limits during those months and federal block-grant funding remains available.
After approval, DHS sends a buy-in file to CMS, which tells Social Security to stop withholding the Part B premium from your check. That process takes 60 to 90 days. SSA issues a retroactive refund for every month of premiums withheld after your coverage start date.
Applying on Behalf of a Parent
An adult child can help with the application, but speaking directly with DHS caseworkers requires formal authorization. You'll need to complete the Designated Representative section on the MPP-1 form (or file a separate DHS Designated Representative Form). A valid Power of Attorney with financial and administrative powers also works.
The Rhode Island Medicare Savings Programs, Extra Help & RIPAE Guide includes a step-by-step walkthrough of the authorization process and a document collection checklist you can share with a parent — useful when you're coordinating paperwork from out of state.
