Access Nevada Medicaid Application for Medicare Savings Programs
There Is No Standalone MSP Application
Nevada doesn't have a separate Medicare Savings Program application form. Instead, you complete the full Medical Assistance application — either online through the Access Nevada portal or on paper using Form 2960-EG. DWSS evaluates you for all MAABD (Medical Assistance to the Aged, Blind, and Disabled) categories at once, which includes QMB, SLMB, and QI.
This trips people up. They search for an "MSP application" or a "QMB form" and can't find one. That's because the same application that covers full Medicaid also covers the Medicare Savings Programs. You don't need to specify which tier you want — DWSS determines which one fits your income and assets.
Access Nevada Online Application
The portal at accessnevada.nv.gov is the fastest route. Select the "Medical Assistance" path and complete the integrated application. You'll enter income, assets, household composition, and Medicare enrollment status.
Two common problems with the portal:
Session timeouts. The system logs you out after a period of inactivity, and you lose unsaved progress. Work through each section without long pauses. If you're gathering documents between sections, write down your answers offline first.
Authorized representative documents. If an adult child is applying on behalf of a parent, include the DWSS Authorized Representative form (Form 2525-EE) with the application. If you can't attach it online, fax it to Southern Nevada at (702) 486-1634 or Northern Nevada at (775) 448-5094, or mail it to the DWSS Processing Center.
Paper, Fax, and In-Person Options
Not everyone can use the portal. Here are the alternatives:
Paper application (Form 2960-EG): Download from the DWSS website or pick up at any district office. Mail the completed form to DWSS Processing Center, P.O. Box 15400, Las Vegas, NV 89114-5400.
Fax: Southern Nevada at (702) 486-1634. Northern Nevada at (775) 448-5094. Fax confirmation pages serve as your proof of submission — keep them.
In-person drop-off: DWSS district offices accept walk-in applications. Major offices include Belrose and Owens in Las Vegas, Henderson, Reno, and Carson City. Rural offices in Elko serve the northeastern counties.
Phone: Call Southern Nevada at (702) 486-1646, Northern Nevada at (775) 684-7200, or toll-free at (800) 992-0900. Expect hold times of 30 minutes to an hour during peak periods.

What to Submit With Your Application
Submitting complete documentation upfront can reduce follow-up requests and delays. Include:
- Copy of your Medicare card (red, white, and blue card) or SSA award letter
- Nevada driver's license or state ID plus a utility bill or lease
- Most recent Social Security benefit statement (SSA-1099) or pension statement
- Bank statements from the prior month for all accounts
- Proof of citizenship or immigration status
DWSS must process your application within 45 days. If they need more documentation, they'll send an information request that lists what is needed and the date it must be received. Respond by the date on the notice; if the information is not received by then, your application may be denied.
After You Submit
DWSS reviews your application against the MAABD criteria. If approved for QMB, coverage starts the first of the month after approval — it's prospective only. SLMB and QI can be backdated up to three months. Either way, Social Security continues deducting your Part B premium for 60 to 90 days while the buy-in processes through CMS. SSA refunds every deducted month in a lump sum once the transmission completes.
The Nevada Medicare Savings Programs & Extra Help Guide includes a complete verification checklist, the application walkthrough for each submission channel, and the timeline from submission to premium refund — so nothing falls through the cracks during the 45-day processing window.
