Wyoming Medicare Savings Program vs Full Medicaid
Two Programs, Very Different Rules
Families in Wyoming routinely confuse Medicare Savings Programs with full Medicaid, and the confusion costs them. A parent who assumes QMB has the same rules as nursing home Medicaid—$2,000 asset limit, 60-month look-back, estate recovery—may never apply for help they're entitled to.
Here's what actually separates them in Wyoming.
Income and Asset Limits
| Medicare Savings Programs | Full ABD Medicaid | |
|---|---|---|
| Individual income limit | QMB: $1,330/mo, SLMB: $1,596/mo, QI: $1,796/mo | $994/mo |
| Couple income limit | QMB: $1,804/mo, SLMB: $2,164/mo, QI: $2,435/mo | $1,491/mo |
| Individual asset limit | $9,950 | $2,000 |
| Couple asset limit | $14,910 | $3,000 |
| $20 general disregard | Yes | Yes |
| Earned income deductions | Yes ($65 + half remainder) | Yes |
These 2026 MSP income standards are significantly higher because they're pegged to 100-135% of the Federal Poverty Level, while full ABD Medicaid tracks the SSI Federal Benefit Rate. WDH compares countable income with the standard after applying allowable disregards. And the asset limit difference is dramatic—nearly five times higher for MSP.
What Each Program Covers
Medicare Savings Programs help with Medicare costs only:
- QMB: Pays Part A premium (if applicable), Part B premium, all Medicare deductibles, coinsurance, and copayments
- SLMB: Pays Part B premium only
- QI: Pays Part B premium only (subject to annual federal funding)
All three tiers automatically trigger Extra Help for Part D prescription drug costs.
Full ABD Medicaid covers a broader range of services beyond Medicare. A person who qualifies for both programs receives the Medicare cost help through their MSP and additional services through full Medicaid, including:
- Dental care
- Vision and eyeglasses
- Non-emergency medical transportation
- Home and Community-Based Services (through the Community Choices Waiver)
- Nursing home care
- Durable medical equipment beyond what Medicare covers

Estate Recovery
This is where the fear lives—and where the distinction matters most.
Medicare Savings Programs: Federal law (42 U.S.C. § 1396p) exempts MSP benefits from estate recovery for dates of service on or after January 1, 2010. Wyoming cannot pursue your parent's house or other assets to recoup QMB, SLMB, or QI payments.
Full Medicaid (long-term care): Nursing home and HCBS waiver costs paid by Medicaid are subject to estate recovery from the probate estate after death.
A parent enrolled only in QMB to pay their Medicare premiums and cost-sharing has zero estate recovery exposure. A parent receiving nursing home Medicaid does.
Transfer Penalties
Medicare Savings Programs: No transfer penalties. Under ECOM POL M1101, giving away assets before applying for QMB, SLMB, or QI does not create a disqualification period. There is no look-back.
Full Medicaid (long-term care): Wyoming enforces a 60-month look-back period. Asset transfers for less than fair market value trigger penalty periods that delay Medicaid coverage. This is where Miller Trusts (Qualified Income Trusts) come in—and where elder law attorneys earn their fees.
Can You Have Both?
Yes. A person can be enrolled in a Medicare Savings Program and also qualify for full ABD Medicaid if their income is low enough. Someone receiving $900/month in Social Security with $1,800 in assets qualifies for both QMB (covering Medicare costs) and full Medicaid (covering non-Medicare services).
If your parent's countable income is above the full Medicaid standard ($994/mo individual) but within an MSP tier's standard (up to $1,796/mo individual), they may qualify for MSP if they meet its Medicare, asset, and other eligibility rules while remaining over the full Medicaid income standard. That's the sweet spot where MSP exists specifically to help—and where estate recovery fears shouldn't stop the application.
