Virginia Medicare Savings Program Documents Needed

What Virginia's System Checks Automatically

Before asking you for paper documents, your local DSS runs electronic verification through three systems: the Asset Verification System (AVS), the State Online Query-Internet (SOLQ-I), and the Federal Data Services Hub. These systems check bank account balances, Social Security benefit amounts, wage records, and other financial data.

If the numbers you attested to on the application are "reasonably compatible" with what the electronic systems return — defined as within 20% of electronic data, or both figures below the eligibility threshold — the caseworker may waive paper verification entirely and approve the application based on electronic records alone.

This means some applicants never need to submit a single document beyond the application itself.

When Paper Documents Are Required

If electronic verification is incomplete or flags a discrepancy, the DSS issues a Verification Checklist (VCL) listing exactly what they need. You get at least 10 days to respond.

The most commonly requested documents:

Identity and Medicare enrollment

  • Medicare card (red, white, and blue card showing Part A and/or Part B enrollment)
  • Government-issued photo ID
  • Social Security card or SSA benefit verification letter

Income verification

  • Social Security benefit verification letter (SSA-1099 or a recent benefit statement from ssa.gov showing gross monthly benefit — not the net deposit amount)
  • Pension award letters or statements
  • VA benefit award letters (specify Aid and Attendance or housebound amounts separately, as these are excluded from income)
  • Any earned income documentation (pay stubs, self-employment records)

Resource verification

  • Bank statements (checking and savings, most recent 1-2 months)
  • Certificates of deposit statements
  • Retirement account statements (IRA, 401(k), Keogh — note whether the account is in active periodic payout)
  • Life insurance policy face value documentation (if combined face value exceeds $1,500, the full cash surrender value is counted)

Other

  • Proof of Virginia residency (utility bill, lease, mortgage statement)
  • If filing for someone else: completed Appendix C (Authorized Representative form)

The Key Form: Appendix D

If applying on paper, the critical supplement is Appendix D — the form for aged, blind, or disabled applicants that captures non-MAGI income, liquid resources, and Medicare enrollment details. Without Appendix D, the application is incomplete for MSP purposes and the caseworker can't evaluate eligibility for QMB, SLMB, or QI.

Online applications through CommonHelp collect this information through the screening questions and don't require a separate Appendix D.

Where to Send Documents

  • Email: [email protected]
  • Fax: 1-888-221-9402 (Cover Virginia fax line)
  • Mail: Cardinal Care Correspondence Center, P.O. Box 1820, Richmond, VA 23218
  • In person: your local DSS office

Email and fax give you a transmission record. If mailing, use certified mail with return receipt — paper documents mailed to local DSS offices have been known to go untracked, and if the 45-day processing deadline passes because a document was lost in intake, proving you submitted it matters.

Processing Timeline

Virginia law requires a decision within 45 calendar days of the application filing date. That clock starts when the application is received — at the local DSS, through CommonHelp, or via Cover Virginia.

In practice, processing times vary widely. Urban offices with high caseloads (Richmond City, Fairfax, Norfolk) sometimes run close to the 45-day limit. Rural offices may process faster.

If 45 days pass without a decision and the delay isn't caused by missing documents from the applicant, you can request a fair hearing through the DMAS Appeals Division for a delayed-decision remedy.

For a printable document checklist organized by verification category, plus a submission tracking log to record what was sent, when, and how, the Virginia Medicare Savings Programs & Extra Help Guide has it ready to fill out.
