Documents Needed for the South Dakota Medicare Savings Program Application
The application form for South Dakota's Medicare Savings Programs is only four pages. The paperwork behind it can take days to assemble. Missing even one document delays the 45-day decision timeline while DSS sends a verification request and waits for your response.
What DSS Requires
Medicare card. A copy showing the Medicare Beneficiary Identifier (MBI) and the effective dates for Part A and Part B. If the card has been lost, SSA can reissue one or provide the MBI over the phone, but having the physical card copied and ready is the simplest path.
Income verification. DSS needs proof of all regular monthly income:
- Social Security award letter (the annual notice SSA sends, or request a current one at ssa.gov)
- Private pension statements
- VA benefit letters
- For earned income: 30 consecutive days of paystubs
The award letter is the key document for most applicants. It shows the gross benefit amount — which is what DSS uses, not the net direct deposit after Part B is deducted.
Bank statements — three full months. This is the document requirement that creates the most delays. DSS requires complete statements for every checking, savings, and investment account covering the three calendar months prior to the application month.
Complete means every page, including pages with only transactions and no summary balances. A four-page bank statement submitted with only the first and last pages triggers a verification request.
Identity and residency. A South Dakota driver's license, state ID card, or utility bill showing a South Dakota address.
The Joint Bank Account Question
For married couples living together, DSS evaluates both spouses' resources against the couple limit ($14,910). Every account owned by either spouse must be documented — both names on a joint account, and any accounts held individually.
A joint account held with a non-spouse (an adult child's name on the account for convenience, for example) can complicate things. DSS may need an explanation of the account ownership and whose funds are actually in it. Having a brief written note prepared explaining the arrangement can prevent follow-up questions.
Submitting Documents
Three channels for document submission:
Online through eaportal.sd.gov — scan or photograph documents and upload them as PDFs. The portal accepts standard file sizes, but families report timeouts when uploading large multi-page bank statements. Splitting files into smaller PDFs helps.
In person at a local DSS office — bring the originals and copies. The worker can verify the originals and keep the copies. DSS offices are in Sioux Falls, Rapid City, Aberdeen, Pierre, and other locations.
By mail — send copies, never originals, to the local DSS office handling the application. Use certified mail or at minimum keep proof of mailing. A lost envelope full of bank statements means starting the document collection over.

What Happens If Documents Are Missing
DSS issues a written notice listing the specific documents still needed and sets a deadline for submission. The 45-day processing clock may effectively pause until the verification is complete.
If the deadline passes without a response, DSS can deny the application for failure to provide verification. This isn't a permanent denial — the applicant can reapply — but it restarts the entire 45-day process.
Documents for Renewal
Annual renewals require updated financial documentation too. DSS mails a pre-populated renewal form and typically asks for current bank statements and any changes in income. Having the same three-month bank statement habit at renewal time prevents a gap in coverage.
